Position: Senior Case Manager Reports To: Senior Program Manager Location: 240 West 167th Street, Bronx, NY 10458 What The Senior Case Manager Does: • Review all documentation establishing tenants' eligibility for program and make file copies., • Create and maintain tenant files., • Conduct initial intake and assessment of tenants and tenants' families' needs and periodic re-assessments., • In collaboration with tenants, prepare initial and periodic revisions of independent living plans including short-term and long-term tenant goals., • Assist tenants in attaining their goals by identifying and locating community resources for tenants and by making referrals to appropriate services both within and outside CAMBA., • Work with tenants to break through barriers to tenant goals and to assist tenants in advocating for themselves and in moving toward self-sufficiency., • Conduct group sessions and/or workshops to assist tenants with independent living skills., • Recommend and implement strategies to persuade tenants to participate more fully in this process., • Monitor tenants' progress toward their goals via regularly scheduled telephone contact and/or face-to-face visits, and document via progress notes., • Participate in case conferences., • Follow-up with tenants and with referral organizations regarding tenant contact and progress with referral organization., • Provide all required information for weekly/monthly/quarterly/annual reports., • May recommend closing of cases in which tenants have: (a) achieved primary goals and have maintained stability for a period of months; or, (b) have not demonstrated a willingness to participate in the process (lost-to-service); or, (c) have become ineligible for services (e.g. moved out of building)., • May act as tenant liaison/tenant advocate with outside organizations regarding such matters as education, healthcare, housing, legal issues, etc., • May schedule appointments for tenant with referral organizations., • May escort tenants to appointments (educational, medical, social service, etc.), • May assist tenants in completing applications for benefits and entitlements, and may process applications on tenants' behalf., • May follow-up with tenants for a period of time after successful completion of their primary goals to assure tenant stability., • May evaluate actual living conditions of tenants through home visits., • May prepare marketing materials for the program. Minimum Education/Experience Required: • Bachelor's degree in Social Work (B.S.W.) or related field and 2 years of relevant experience. Other Requirements: • Obtain “Criminal History Checks” (CHC) fingerprinting through the New York State Office of Mental Health (OMH) and the Justice Center for the Protection of People with Special Needs (Justice System) and Staff Exclusion List (SEL) clearance., • Ability to maintain fingerprint clearance throughout the duration of employment., • As a Mandated Reporter, you must immediately report suspected incidents/cases of abuse and neglect to the Vulnerable Persons Central Register (VPCR) Hotline at 1-855-373-2122 and complete a CAMBA Incident Report., • Flexible hours may be needed occasionally to provide shift coverage until 8 pm. Compensation: $56,000-$60,000 annually When salary ranges are listed, the range would represent the low and high end for the applicable position & program. The salary offered would be based on various factors unique to each program and candidate. This includes but is not limited to experience, education, budget and/or program size, internal equity, skills and other factors that may be required for the position and organization. Must be able to stabilize and promote self-sufficiency and optimum health for people living with HIV/AIDS, Mental illness and/or substance use disorders. Manage a case load of 20 to 25. Provide advocacy as well as coordinate services through home visits. SPECIFIC DUTIES & RESPONSIBILITIES: • Provide substance abuse intervention, counseling, educational services, and case management services to tenants., • Evaluate and recommend treatment plans for tenants with mental health and chemical dependency problems after a one-to-one interview., • Provide regular, individual, and group counseling services., • Establish community and agency affiliations to strengthen the delivery of services for tenants, including medical & detoxification when necessary., • Attend case review conferences with HRA/HASA and provide written evaluations regarding tenant’s drug problems and money management issues., • Present case load weekly at chart review., • Meet for supervision with Assistant Director bi-weekly., • Orient tenant’s to the program upon admission., • Maintain up-to-date tenant’s charts, case notes, service plans and psychosocial., • Attends all in-service training., • Responsible for collection of tenants monthly rent., • Set a relaxed tone and social environment (by role modeling), • Other duties as assigned REQUIREMENTS: EDUCATION AND EXPERIENCE KNOWLEDGE, SKILLS AND ABILITIES: High School Diploma, one to two years of experience in counseling or two (2) years of Odyssey House experience and a High School Diploma, or Bachelor’s Degree in Human Service.
