The responsibilities include: Serving as a member of the location's team committed to helping young people build on existing assets to make positive choices about their health and well-being, academic achievement and interaction with others. Coordinating the implementation of Individual Service Plans (ISP) for each youth in accordance with agency ISP planning policies and procedures. Establishing and monitoring the methods used to teach the youth new skills as per individual ISP’s and works with program staff to provide services in accordance with established methods. Coordinating and linking each youth to off-site support services as defined in ISP. Planning the transportation for the youth’s participation in off-site services. Completing all case management documentation and reports and maintaining individual youth files. Reviewing medication logs daily. Conducting weekly audit of medication logs to ensure compliance with medication administration policy. Implementing behavior modification plans established by the team to address discipline issues and the consumer’s lack of adherence to program rules, as needed. Prospective applicants must have a Bachelor’s Degree or Master’s Degree in social work or human services related field; one to three years of experience working with young adults and adolescents, particularly at-risk individuals; excellent organizational skills; demonstrated leadership ability and sound judgment; ability to work effectively as a member of a team; ability to work collaboratively with community resources; excellent interpersonal, verbal and written communication skills; and a valid driver’s license.
