Occasionally, pizza sauce weighing 30 pounds is carried from the storage room to the front of the store. Trays of pizza dough are carried three at a time over short distances, and weigh approximately 12 pounds per tray. Pushing To move trays which are placed on dollies. A stack of trays on a dolly is approximately 24- 30 and requires a force of up to 7.5 pounds to push. Trays may also be pulled. Climbing Team members must infrequently navigate stairs or climb a ladder to change prices on signs, wash walls, perform maintenance. Summary/Objective: To help maintain the child/youth in the home, school, and community, the Home Based Crisis Intervention (HBCI) team delivers intensive crisis intervention to children and families within Jefferson County. The HBCI Program aims to serve children/youth ages 5 to 20 years at imminent risk of psychiatric hospitalization or admission to a group treatment program, or at risk for a rapid readmit to such settings. Under the supervision of the HBCI Clinical Supervisor, the HBCI Interventionist responds to referrals and provides treatment to address the clinical and complex needs of the child/youth and their family/caregivers. In addition to providing direct crisis intervention and stabilization, the Interventionist provides case management services linking the family with additional short and long-term available services. TCRP provides 24/7 monitoring and supervision, as well as intensive crisis treatment and support for the youth, family, and caregivers to facilitate the youth’s successful return home and/or to the community. The TCRP Clinician is responsible for the provision of direct clinical services to youth and their families, conducting assessments, developing treatment plans, and delivering therapeutic interventions. They work collaboratively with a multidisciplinary team, including psychiatrists, social workers, and other healthcare professionals, to ensure comprehensive care. CHJC and its dedicated Care Coordinators strive to improve the responsiveness, accountability, and navigation of the complex medical, behavioral, and social service system. The Care Coordinator is responsible for conducting strength-based assessments, identifying and submitting referrals for needed services/community supports, and providing advocacy and support to individual members. To ensure the member receives quality services and maintains optimum healthcare without barriers, the Care Coordinator communicates with each member and their care team. Services are provided by a comprehensive network of provider agencies, delivering an array of medical, self-help, social, supportive, and rehabilitative services. Based on the individual needs of the member, individualized service plans are developed. Essential Functions: 1. Mobile Crisis Services are provided to children and adults who are experiencing, or at imminent risk of experiencing, a behavioral health crisis. A behavioral health crisis includes instances in which a person cannot manage their primary psychiatric and/or substance use related symptoms without de-escalation or intervention. The mission of the Clinician and Mobile Crisis Team is to deliver person-centered, trauma-informed, culturally linguistic competent services that are high quality, effective, and integrated community-based services promoting resilience, rehabilitation, and recovery. These services are available telephonically and face-to-face in the community and are aimed at providing immediate support and offer alternatives to hospitalization when appropriate. Essential Functions: • Provides mobile and telephonic crisis response services throughout Jefferson and Lewis counties and delivers services in the least restrictive available setting., • Interacts with local emergency systems (e.g. 911, local crisis hotlines), hospitals, law enforcement, and certified Peer Advocates to include co-response in order to provide safe and coordinated crisis response., • Provides immediate, short-term clinical interventions to stabilize individuals, their families and/or caregivers in the home, community, office, and natural environments., • Employs flexible, person-centered interventions, calming techniques and models positive, soothing behaviors., • Evaluates and assesses the holistic needs of individuals and their family, including the predictors of safety risks in the home and community., • Utilizes effective clinical judgement utilizing evidence-based clinical tools (SBIRT, C-SSRS, PHQ-9, QPR) and safety plans as appropriate., • Assists recipients through the introduction and reinforcement of crisis management skills and identification of community resources and supports., • Coordinates with recipient’s current treatment provider(s) and other identified supports to resolve the current crisis and to aid in preventing future crises., • Works in consortium with the Crisis Case Manager to provide follow-up services and efforts to appropriately link and refer individual to support services within the community., • Values and protects the rights, privacy, and confidentiality of recipients, with the understanding that whenever necessary, contact will be made with collaterals who may have information to inform the assessment., • Collects data and maintains client records according to agency and standard guidelines. The clinician works in a community environment, including client homes, counseling spaces, and other community settings. Exposure to noise and occasional physical activity may occur. Travel between community locations is required. May have contact with service recipients that are agitated and/or confused. Some risk is involved working with service recipients with serious mental illnesses. The target population is exclusively families who have finalized the adoption of their child(ren), families with legal guardianship of their child(ren), with or without a KinGap agreement, and families with formal or informal custody agreements. The program model is trauma-informed, child-centered, and family-focused, and culturally responsive. The model takes a strength-based approach to family engagement and is non-judgmental and affirming. Program services are evidence-based, easily accessible, as well as flexible and adaptable to the family’s needs. The program model is designed to: · Promote permanency for post-adoptive, post-guardianship, and kinship families. · Provide assistance to families so children may be cared for in their own homes with their adoptive parent(s), legal guardian(s), or kinship caregiver(s); and · Strengthen post-adoptive, post-guardianship, and kinship families to avoid foster care or other out-of-home placements. The FES works as a member of the treatment team to deliver high intensity case management to assigned youth and families. Essential Functions: 1. Assists in gathering essential documentation for program enrollment, 2. Collaborates with clinical staff to completes required assessments and work with the family to create treatment plans/goals, 3.
