Job Description The Contractor shall provide Contract Facility Manager/Building Manager (CFM) support services to the assigned GSA Facility, Field Office or Service Center. Services include: • Inspection and management of federally owned facilities within the Field Office, including maintenance and operational inspections in accordance with applicable GSA property management principles, SOPs, and guidelines., • Inspection and management of leased locations within the Field Office in accordance with applicable GSA lease administration principles, guidelines, and SOPs., • Construction Management (CM) and Project Management (PM) services for repair and alteration projects in both leased and federally owned facilities., • Projects may range from under SAT to the prospectus project limit., • Duties include project estimating, scoping, development, design and/or design review (based on project complexity), inspections, punch list preparation, and project closeout., • The Contractor shall provide a well-trained and experienced candidate meeting the minimum qualifications outlined in this SOW., • The COR will generally be the GSA Field Office Manager or a delegated representative., • The CFM shall report directly to the COR and follow daily schedules and assignments as directed. 1. GENERAL REQUIREMENTS 1.1 Employment and Supervision 1.2 Professional Conduct • The CFM must maintain professional dress and appearance., • The CFM may serve as the primary point of contact representing GSA to tenant agencies. INSPECTION OF WORK IN FEDERAL FACILITIES The CFM shall: • Inspect construction and building maintenance contracts (janitorial, cafeteria, mechanical, elevator, and related services)., • Follow GSA PBS Program Guides and attached guidelines., • Document findings using approved GSA inspection report formats., • Conduct follow-up inspections to verify corrective actions. 3. Review contract specifications prior to inspection., 4. Draft deficiency notices and cure letters for GSA signature. All formal correspondence must be issued and signed by a GSA employee. The CFM may coordinate meetings with contractors when authorized by the COR. 3. INSPECTION OF WORK IN LEASED LOCATIONS Under direction of the LAM, LCO, or COR, the CFM shall: 1. Inspect leased facilities to ensure lessor compliance with lease terms. • Review the GSA Lease Administration Guide and applicable SOPs., • Focus inspections on tenant service and customer satisfaction issues. 3. Prepare lease inspection reports and draft correspondence for GSA signature., 4. Coordinate professionally with tenants and lessors. The CFM shall not formally direct lessors; only GSA personnel may issue formal direction. 4. CONSTRUCTION MANAGEMENT (CM) AND PROJECT MANAGEMENT (PM) The CFM shall perform CM and PM services in accordance with GSA PBS Program Guides and Project Management Guidelines. 4.1 Project Responsibilities ● Prepare cost estimates and scopes of work. • Manage budgeting and financial documentation., • Monitor construction activities., • Participate in pre-final and final inspections., • Perform basic CAD drafting and specification writing as required., • Serve as on-site representative during construction activities., • Review design documents and submittals., • Maintain defect and omission lists., • Verify contractor payment requests (value in place and stored materials)., • Prepare written recommendations for acceptance., • Assist with claims analysis and dispute documentation., • Enforce OSHA safety regulations. The CFM shall NOT: ● Authorize contract modifications. ● Make final decisions on claims or disputes. All formal directions must be issued by authorized GSA personnel. 5. CUSTOMER SERVICE RESPONSIBILITIES The CFM shall: • Improve customer and client agency satisfaction., • Maintain and preserve real property assets., • Participate in GSA Tenant Satisfaction Surveys., • Coordinate reimbursable work and ensure funding is in place., • Monitor operating costs., • Serve as property management point of contact for space modifications., • Document inspections and prepare reports for GSA review and signature., • Immediately report dangerous conditions or accidents to the COR. 6. MINIMUM QUALIFICATIONS The proposed CFM must: 1. ESSENTIAL DUTIES AND RESPONSIBILITIES The duties and responsibilities of the Residence Program Specialist will include but are not limited to the following: • Provides care and assists with personal development and independence of the people we support., • Ensures that all measures for safety and well-being are maintained at highest level., • Provides training for the people we support in activities of daily living and in development of self-help and social skills., • Maintains a personalized, homelike atmosphere in the residential facility., • Attends weekly staff meetings as well as any other assigned meetings and in-service trainings., • Prepares concise written reports including Incident Reports, Progress Notes and other reports as assigned., • Responds to crisis situations and medical emergencies, including accidents by taking quick and decisive action., • Ensures actions are conducted in a manner that prevents abuse, and reports observations of co-workers who may be abusive in accordance with established procedures., • Administers appropriate first aid based on visible symptoms and medical history of the people we support in accordance with established guidelines and accepted first aid practices., • Summons police, fire, ambulance or other emergency personnel if situation warrants it., • If an emergency has caused a person we support to exhibit behavior that is threatening or dangerous, may have to utilize strategies for crisis intervention in accordance with OPWDD approved Behavior Management guidelines., • Provides transportation for the people we support in agency vehicles to and from work, shopping, programs, restaurants, movies, doctor’s appointments, etc., • Assists with meal planning and preparation as well as with purchasing of food and supplies., • Demonstrates an understanding of and implements behavioral programs., • Participates fully as a member of interdisciplinary plan., • Maintains an ongoing informal exchange of information. Maintains written logs for recording daily activities., • Performs housekeeping, cooking, laundry, light maintenance and light preventative maintenance duties as needed to maintain a safe and homelike environment., • Supervises any other activities as may be necessary to maintain quality of the program., • Dispenses prescribed medications to the people we support after successful completion of the medication administration course, and in line with Agency protocol. AMAP’s are supervised by residential nurse in all areas related to medication administration., • Completes all activities related to skill buildings as outlined in individual program plans, • Provides support around personal hygiene, i.e. toileting, bathing and changing as needed., • Handles all personal documents of the people we support in a secure and confidential manner., • Performs related duties as requested. Job Description SUMMARY This position is responsible for the provision of clinical counseling services in the areas of chemical dependency and mental health, providing holistic and comprehensive interventions, resources and referrals; services are provided primarily in the community. This person directly reports to the Program Manager. ESSENTIAL DUTIES AND RESPONSIBILITIES; Core duties and responsibilities include the following. Other duties may be assigned. • Collects and analyzes assessment data which includes substance use, mental health and co-occurring disorders., • Completes intake process and psychosocial assessments, arriving at treatment recommendations and level of care decisions that are appropriate to participant’s level of functioning and medical necessity., • Develops initial and comprehensive individualized treatment plans based on participant’s needs, using participant’s words, and that have measurable objectives, within established timelines, based on assessment, psychosocial, participant’s assessed stage of change, and in consultation with participant and treatment team as appropriate., • Provides individual and group counseling based on treatment plan and participant centered needs., • Conducts evidence based practices to the individual needs of the participant (EBPs include, but are not limited to, Cognitive Behavior Therapy, Motivational Interviewing, Matrix Model, ---Trauma Focused Cognitive Behavioral Therapy)., • Identify participants with urgent mental health conditions (e.g. suicidal, homicidal, psychotic, etc.) and make appropriate referrals to available emergency or psychiatric treatment facilities., • Is attuned to the presence of trauma indicators and projects safety and trust in the professional relationship., • Have the ability to provide grief and crisis intervention services., • Be able to address social needs of participants and utilize area referral sources for needed specialty care, hospitalization, or advance psychiatric assessments and treatments., • Encourages family involvement when appropriate., • Conducts discharge planning with the client., • Acts as liaison with networking/referring agencies, e.g.; Courts, Parole, Probation, ACCESS, etc., • Recognizes, responds to, and defuse volatile or dangerous situations as allowable; when in doubt always call for assistance and/or 911., • Problem Solving – Identifies and resolves problems in a timely manner; Works well in group problem solving situations., • Technical Skills – Assesses own strengths and weaknesses; Pursues training and development opportunities.
