The Medspan program provides patient education and care coordination around HIV, hepatitis C, and associated conditions such as hepatitis B and sexually transmitted infections (STIs). Medspan team members support patients through medication adherence support, discharge planning, and connection to care services. The Medical Director works across the Correctional Health Service Medicine service, with MedSpan staff and with other interdisciplinary Correctional Health Services (CHS) staff in all facilities to provide, improve, and evaluate patient centered infectious disease care. Responsibilities include: (Detailed Task) Clinical Responsibilities: • Identify individuals admitted to correctional facilities with infectious disease (ID) needs through the utilization of electronic health records, report management systems, and various tracking tools., • Provide direct patient care to individuals experiencing acute or chronic infectious disease conditions, including but not limited to HIV, hepatitis C and B, sexually transmitted infections, tuberculosis, skin and soft tissue infections, wounds, and systemic bacterial processes., • Deliver clinical consultation and guidance related to infectious diseases to site medical directors (SMD), infection control teams, and clinical staff. Duties & Responsibilities Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re-entry services, and relationships with multiple training institutions throughout the city. The Unit Chief is the mental health administrative lead in each facility or designated area, including general population and therapeutic housing units. Under the direction of the Director of Mental Health Administration, this full-time position coordinates, and helps manage, mental healthcare delivery in one or several of the jail facilities in New York City. This includes ensuring provision of services within the timeframes and quality benchmarks outlines by CHS MH policies, providing administrative support to staff who are assigned to their clinical area, maintenance of patient records and compliance with monitoring requirements. S/he will act as the principle administrative liaison between mental health disciplines, as well as pharmacy, medical and nursing. As a member of the facility management team, the Unit Chief will collaborate closely with the Department of Correction (DOC) leadership and CHS Operations in order to ensure service delivery and meet patient needs. A major component of this position will be providing effective, inspiring, and clinically-informed administrative leadership and organization. Possession of a valid license and current registration to practice in a mental health discipline issued by the New York State Education Department (NYSED); and, 2. Three (3) years of progressively responsible experience in a health care setting or regulatory agency administration, with an emphasis on development and evaluation of mental health delivery services; one (1) year of which must have been in an administrative, managerial or supervisory capacity. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, clinics in each of the facilities, substance use treatment, medication assisted treatment (MAT), integrated re-entry services that includes alternatives to incarceration, and relationships with multiple training institutions throughout the city. Furthermore, CHS has been engaged in an effort to better identify people with neurodevelopmental/neurocognitive disorders and to utilize psychological testing to connect people to services in the community (e.g., with OPWDD). Responsibilities: Under the supervision of a Senior/Supervising Psychiatrist in the Mental Health Service of CHS, this position requires the provision of high quality, team-based psychiatric services including psychopharmacologic interventions, for individuals incarcerated in the New York City jail system. Each of the facilities in the jail system has unique patient population and environmental characteristics; each psychiatrist will be matched with the population that most closely meets their clinical expertise, desired work location and schedule and the clinical needs of the service. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical. Responsibilities Include: • Provide psychiatric care, including admission evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs., • Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information., • Evaluate patients for the need for a higher level of care, including hospitalization, and refer to a higher level of care as clinically appropriate. If a patient is hospitalized, maintain contact with the hospital psychiatric staff to ensure continuity of care., • Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare services. The Evening Front Desk Receptionist plays a key role in maintaining a safe, organized, and welcoming environment while providing administrative, logistical, and customer service support to programs and staff operating in the facility. Key ResponsibilitiesFront Desk & Customer Service • Serve as the primary point of contact for visitors, participants, and staff during evening hours, • Greet and assist community members, program participants, and guests in a professional and welcoming manner, • Answer and direct phone calls, inquiries, and emails, • Provide general information about programs, services, and schedulesAdministrative Support, • Support program staff with administrative tasks such as copying, printing, scanning, and filing, • Assist with participant sign-in processes and documentation when needed, • Maintain organized records and ensure forms are completed accurately, • Support administrative staff with data entry and general office tasksFacility & Operations Support, • Monitor evening use of classrooms and shared spaces to ensure scheduled programs run smoothly, • Assist with room set-ups and breakdowns for meetings, classes, and community events when needed, • Ensure shared spaces remain organized and presentable, • Report any facility issues or maintenance concerns to the Operations or Administrative teamSafety & Security, • Monitor building access during evening hours and ensure proper visitor protocols are followed, • Maintain awareness of who is in the building and support safe facility operations, • Respond appropriately to minor issues and escalate concerns to supervisors as neededCoordination & Communication, • Maintain clear communication with program staff, administrative staff, and site leadership, • Support coordination of evening activities and help ensure programs stay on schedule, • Document important incidents, updates, or facility concerns during the shiftQualifications, • High school diploma required, • Previous administrative, receptionist, or front desk experience preferred, • Professional and welcoming demeanor, • Strong organizational and time management skills, • Attention to detail, • Strong interpersonal and problem-solving skills, • Proficiency in Microsoft Office (Word, Excel, Outlook), • Familiarity with office equipment such as printers, scanners, and fax machines, • Ability to manage multiple priorities in a fast-paced environment, • A collaborative, mission-driven workplace, • Opportunities to support programs that positively impact communities across Queens, • Queens Community House supplements the New York State Paid Family Leave benefit to ensure eligible employees receive full pay during approved leave, • Personal time off, including sick and vacation time Queens Community House maintains a Drug-Free Workplace.
