Job DescriptionDescription: Summary/Objective: CHJC’s Family Permanency Program (FPP) is funded through the New York State Office of Children and Family Services and serves families in Jefferson, Lewis, Oswego and St. Lawrence Counties. The target population is exclusively families who have finalized the adoption of their child(ren), families with legal guardianship of their child(ren), with or without a KinGap agreement, and families with formal or informal custody agreements. The program model is trauma-informed, child-centered, and family-focused, and culturally responsive. The model takes a strength-based approach to family engagement and is non-judgmental and affirming. Program services are evidence-based, easily accessible, as well as flexible and adaptable to the family’s needs. The program model is designed to: · Promote permanency for post-adoptive, post-guardianship, and kinship families. · Provide assistance to families so children may be cared for in their own homes with their adoptive parent(s), legal guardian(s), or kinship caregiver(s); and · Strengthen post-adoptive, post-guardianship, and kinship families to avoid foster care or other out-of-home placements. The FES works as a member of the treatment team to deliver high intensity case management to assigned youth and families. Essential Functions: 1. Assists in gathering essential documentation for program enrollment, 2. Collaborates with clinical staff to completes required assessments and work with the family to create treatment plans/goals, 3. Monitors progress toward treatment plans/goals, 4. Schedules/facilitates regular treatment team meetings, 5. Identifies areas of concern/barriers to goal achievement, discusses immediate needs and concerns with youth and caregivers, assists in the development of safety/emergency plans, 6. Job DescriptionDescription: Summary/Objective: The Clinical Supervisor is a licensed, master's level clinician, who provides direct oversight of CHJC’s newest program, Home Based Crisis Intervention (HBCI). The HBCI Program aims to serve children/youth ages 5 to 20 years at imminent risk of psychiatric hospitalization or admission to a group treatment program, or at risk for a rapid readmit to such settings. To help maintain the child/youth in the home, school, and community, the HBCI team delivers intensive crisis intervention to children and families within Jefferson County. The Clinical Supervisor is responsible for the oversight and provision of HBCI services, plays a vital role in providing clinical and administrative supervision to Crisis Interventionists, and serves as a clinical decision maker for youth enrolled in HBCI. CHJC and its dedicated Care Coordinators strive to improve the responsiveness, accountability, and navigation of the complex medical, behavioral, and social service system. The Care Coordinator is responsible for conducting strength-based assessments, identifying and submitting referrals for needed services/community supports, and providing advocacy and support to individual members. To ensure the member receives quality services and maintains optimum healthcare without barriers, the Care Coordinator communicates with each member and their care team. Services are provided by a comprehensive network of provider agencies, delivering an array of medical, self-help, social, supportive, and rehabilitative services. Based on the individual needs of the member, individualized service plans are developed. Essential Functions: 1. Assist members to identify strengths and develop problem solving strategies to further enhance those strengths, 2. Essential Functions: The major responsibilities of this position include but are not limited to the following: • Respond to each referral within 48-hours., • Meet with each youth and family/guardian to discuss program options, service provision, and alternative service options., • Conduct all admission paperwork and ensure all necessary consents/releases are signed., • Collaborate with already established providers such as care managers, therapists, and psychiatric providers., • Provide intensive crisis intervention and supportive counseling to the youth and family., • Participate in team meetings., • Work with community agencies, organizations, and systems for long-term planning and support of the youth and families served., • Make referrals to mental health services and other programs as needed., • Provide thorough documentation regarding all case related activities including but not limited to; face to face contacts, phone calls, case conferences, clinical interventions, noted incidents, etc., • Represent CHJC as a liaison with various private as well as public service providers and resource agencies., • Complete all CHJC and New York State mandated reports and forms in a timely fashion., • Continuously promotes the program and provides community education., • Participates in an after hours on-call rotation. Work Environment: Office environment will require occasional travel to recruitment events, local schools, conferences and/or meetings and moderate travel within the community to include off-site/in-home visits. May have contact with service recipients that are agitated and/or are in crisis. Some risk involved working with service recipients with mental illness. Works in an office setting with a controlled temperature environment. Occasional exposure to inclement weather conditions may occur depending upon assignments. Equipment: This position is required to maintain a working knowledge of related office equipment including personal computers and printers, audio-visual equipment, telephone systems, copiers, fax machines, etc. Physical Demands: Must be able to sit, stand, walk, lift, carry, push/pull, climb, bend, and stoop. Must be able to perform fine motor skills, read, and type. Must be able to sit for long periods of time. Position Type/Expected Hours of Work: Full-time, traditional hours and non-traditional hours. This position is required to work on-site, and offsite/in home visits. Requirements: Required Education and Experience: Bachelor’s Degree in a Human Services field, with at least two years relevant experience in community-based mental health or case management is required. As this program is seeking applicants possessing a Master’s Degree in a clinical field (i.e., Mental Health Counseling or Social Work), those possessing a Master’s Degree and/or those as licensed clinicians will receive preference. Skills/Abilities/Knowledge: Strong organizational abilities and office skills are essential for this position. This person must also have excellent people skills and be a team player. Ability to work with diverse populations. Must demonstrate effective and excellent oral and written communication skills and the ability to work cooperatively with others. Must be detail-oriented and possess excellent time-management skills. Requires self-direction, tact, diplomacy, and the ability to be clear, courteous, and professional. Able to react to change productively and handle other duties as assigned. Additional Eligibility Qualifications: Must maintain privacy of confidential information.
