Job Description Patient Navigator, 2025-2026 AmeriCorps Summary/Objective Ronald McDonald House New York (RMH-NY) is partnering with AmeriCorps to expand access to care and improve health outcomes for medically complex children and their families. As part of New York State’s Medicaid Waiver initiative, RMH-NY is seeking AmeriCorps members to serve as Patient Navigators. In this role, Patient Navigators will: • Conduct screenings for Health-Related Social Needs (HRSNs), • Facilitate referrals and connect families to essential services This is a full-time (1,700 hours) or half-time (900 hours) AmeriCorps opportunity for one year, beginning in late October 2025. Members receive a living allowance and education award, with stipends of $25,384 (full-time) or $13,483 (half-time) for the full term of service. Through this experience, members will gain professional training in population health and social service navigation while making a direct impact on the lives of children and families facing complex health challenges. Role/Responsibilities Essential Functions: • Understand and define health disparities and social determinants of health (SDoH), • Conduct outreach calls, social needs screenings, and eligibility assessments, • Communicate effectively with cultural competence and sensitivity, • Support community canvassing and partnership-building, • Commit to learning, serving, and growing through hands-on experience in Population Health, • Conduct comprehensive social risk assessments, • Identify supportive services such as mental health, housing, food assistance, and other resources based on assessment results, • Manage, organize, and document interactions and interventions as directed, • Shadow and support Community Health Workers and Referral Coordinators, • Serve as a resource for staff, families, guests, volunteers, donors, and other community members, • Perform other duties as assigned by supervisor AmeriCorp Service Member Benefits • As AmeriCorps members, Patient Navigators will:, • Receive a monthly living stipend, education award, health coverage, and childcare assistance (if eligible), • Participate in ongoing professional development, networking, and service learning opportunities through RMH-NY, • Attend virtual trainings conducted by AmeriCorps, • Service Commitment:, • Full-time = approximately 32 hours per week, • Half-time = approximately 16 hours per week Qualifications, Competencies, and Benefits • High School Diploma or equivalent required; Associate’s or Bachelor’s degree in a human services or related field preferred, • Bilingual skills strongly preferred, • Knowledge of or experience with Community Health Worker practices to promote patient participation and self-care management, • Strong communication and interpersonal skills, with the ability to establish trusting relationships across diverse populations, • Empathy, compassion, and sensitivity to the emotional needs of families coping with a child’s illness, • Excellent organizational skills and attention to detail, with the ability to manage multiple priorities in a fast-paced environment, • Cultural competence and awareness of social justice issues, with a commitment to providing equitable, culturally-responsive care, • Basic computer skills required Additional Equal Employment Opportunity Ronald McDonald House New York believes that all employees are entitled to Equal Employment Opportunity. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines., • The office for this job is located in the same facility where RMH-NY provides temporary accommodations to pediatric patients and their families while the patients are undergoing active medical treatment at area hospitals. For some RMH-NY guests, medical treatment may include radiation therapy. While radiation therapy is never administered at RMH-NY, there is the potential for some radioactive material to be present in guests who have undergone radiation therapy. Based on radiation measurements performed throughout RMH-NY, it is not expected that individuals working at RMH will receive radiation doses that would exceed the general public limit (100 millirem per year)., • The noise level in the work environment is moderate to loud. Noise levels often increase with families and children who have temporary residence in the building as well as guests and musical performers for special events., • This position requires flexible schedule at times to meet the needs of the families.
