Job Description Summary: The Court-Based Mental Health Navigator Care Manager supports individuals involved in the criminal justice system who have mental health or behavioral health needs. The position provides screening, outreach, and care coordination to help connect individuals with appropriate community services. The Navigator works closely with courts, service providers, and community partners to reduce barriers to treatment and promote engagement in care.
