You will assist them in maintaining community tenure and securing housing while fostering self-sufficiency and independence. The role includes promoting the usage of community services, supporting independence in travel, fiscal management, and healthcare, and providing interventions to reduce barriers to service access. You will collaborate with other treatment providers, participate in the development of Individual Rehabilitation Plans, and document progress and observations in client records. Additionally, you will assist individuals in developing social skills and daily living skills to promote community integration. What Does This Position Look Like? In this role, you will work directly with individuals to help them navigate and utilize community resources and services. You will actively promote independence, providing interventions to remove barriers that may hinder clients' ability to thrive in the community. Your work will involve developing individualized plans to support rehabilitation and recovery, collaborating with other providers, and maintaining detailed documentation of your interactions and the services delivered. By assisting clients in gaining the skills needed for social interactions and daily living, you will play a vital role in their path toward greater independence and community participation.
