JCCGCI’s Mental Health Services provide therapeutic and social-emotional support to youth and families affected by community violence, aiming to strengthen connections to community networks and promote culturally grounded behavioral change. The program offers individual and group therapy, supports trauma recovery, and addresses issues like depression, anxiety, and PTSD. It also works in coordination with other programs to connect participants to resources, build the capacity of local providers, and foster community-led approaches to public safety and mental wellness. Job Description Position Title Wellness Coordinator About West Side Federation for Senior and Supportive Housing (WSFSSH) The West Side Federation for Senior and Supportive Housing, Inc. (WSFSSH) is a community‐based organization whose mission is to provide safe, affordable housing with supportive services within a residential setting which enhances the independence and dignity of each person. WSFSSH develops and manages housing and provides social services for low‐income older persons, many of whom live with mental illness and/or have experienced homelessness. Our 440 staff members serve more than 2600 residents in 30 buildings located throughout Manhattan and the Bronx. About WSFSSH’s Wellness Initiative WSFSSH’s Wellness Initiative seeks to create a culture of wellness within our residential developments for older adults to facilitate the adoption of healthy habits that will improve their overall physical, emotional, and mental well-being. We aim to identify, develop and implement best practices to improve health outcomes for older adults, including and particularly for those with histories of homelessness, mental illness and/or substance abuse. An overarching goal of the initiative is to increase our resident connections to high quality primary and preventive health care so as to encourage better resident health management and quality of life through reduced emergency room visits and hospitalizations. The initiative will focus on utilizing appropriate primary and preventive medical care, managing chronic conditions (including medication management), and expanding resident knowledge of healthy habits such as increasing physical activity and consuming more nutritional food. Position Overview The Wellness Coordinator report to the Director of Clinical and Social Services and is responsible for the overall coordination of residents’ health-related services at the site level. The Coordinator must be capable of comprehending complex public health issues and have the ability to develop programs and strategies to engage residents in taking a more proactive approach to their health. The Coordinator builds relationships with the residents and their care team (physicians, nurses, social workers, etc.) to coordinate and ensure the provision of health-related services. The Coordinator assists the residents and their social workers with navigating health-related systems, access issues, and service coordination. The Coordinator acts as a liaison to external health agencies and serves as a conduit for the transmission of health information between WSFSSH’s social work team and the health care providers. Reporting to the Senior Program Manager of Bronx HOPE, the Peer Specialist will participate in a special project with the HOPE team and the NYPD. Job Description Overview Employed by PAGNY for The Division of Correctional Health Services (CHS) of New York City Health + Hospitals (H+H) is responsible for the delivery of medical care, mental health care, and discharge planning in the jail system. Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. This service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re- entry services, and relationships with multiple training institutions throughout the city. Responsibilities Patient Care Associates (PCAs) work under the direction of a licensed nurse or other medical staff to provide patients with care and support. PCAs perform venipuncture, finger-sticks, HIV Pre and Post Test Counseling, Rapid HIV Testing, responding and assisting with emergencies, facilitating telehealth encounters, and various clerical and health-related duties associated with patient care, ADLs, and contributing to the safety, care, and comfort of patients. • Facilitate Telehealth encounters as needed., • Administer total care (bed bath, partial or complete)., • Assist with patient’s hygiene – including skin care, mouth care, shaving, etc., • Transfers patients – in and out of bed, stretcher, and wheelchair., • Positions patients and carry out a range of motion exercise., • Prepares patients for meals and assists with feeding as needed., • Assists with unit patient admissions and transfers., • Measures & records patients’ vital signs (such as height, weight, temperature, blood pressure, pulse, respirations, etc.) in the electronic health record., • Perform EKGs as required and follow outlined procedures for submitting abnormal EKGs., • Perform dry sterile dressing changes and assist with other patient treatment., • Administers hot and cold applications to patients, under supervision., • Perform blood draws and urine testing., • Chaperone staff in all clinical areas when requested., • Responding to/addressing medical emergencies, such as falls, heart attacks, strokes, burns, and recovering post-operative patients., • Provide clinically appropriate care as directed by the Registered Nurse and/or Physician., • When clinically indicated, provide care, such as CPR, AED, prevent shock, control severe bleeding, prevent spinal damage, etc., • Monitor patients’ condition and keep them stable while en route to the clinic., • Aid in preparing patients for transfer to the emergency units or hospitals., • Document the medical treatment that is given to the patient., • Keep the equipment clean after use; check and replace damaged and used supplies., • Collaborate with the pharmacy to replenish supplies. Position Summary The Program Assistant will provide general support to SAYA’s community school programming at Richmond Hill High School (RHHS). They will be responsible for supporting various administrative duties, including but not limited to: data entry and collection; purchase orders and deliveries; office organization; outreach support; and other items as needed. Supervisor: Community School Director Responsibilities: Events and Activity Support * Support team members in coordination of club-based and larger school-wide events before, during, and after events, including but not limited to tasks such as: * Outreach to food, rental, and supply vendors; * Communication of events to students, families, school staff, partners, & vendors; * Liaison with school staff regarding space, supplies, and other needs; * Navigation of internal workflow of event logistics and timeline structure; * On-site event support such as attendance-taking, event setup, managing stations and flow, volunteer coordination, and event breakdown * Comfort with workshop facilitation, as well as supporting lead workshop facilitators during school-day and afterschool activities Data Entry and Collection * Collect and enter various programmatic information into databases, including but not limited to: attendance, incentives, expenses, trip forms, and other items as needed * Maintain organization of program documentation and outreach materials * Administer collection and tracking of program registration, surveys, and other documentation Purchase Orders and Deliveries * Manage purchase orders, receipt collection, and expense documentation * Liaise with vendors regarding progress of purchase orders, invoices and receipts * Comfort with pick-up, delivery, and distribution of food and supplies during in-school and afterschool clubs/events, including transfer of items to alternative locations Office Organization * Ensure daily upkeep of team offices, including but not limited to: file and cabinet organization, supply maintenance and restock, general office organization * Track and organize tech and incentive items in office and on digital spreadsheet Additional Outreach Support * Chaperone trips which may be held during school day, after school, evenings, weekends * Support with organization, outreach, and distribution of student eyeglasses and other resources * Provide support with flyer creation, distribution and outreach for events and other initiatives * Assist team with photo and video documentation and developing content for social media * Attend weekly meetings with supervisor and team members * Other duties as needed and assigned by the supervisor.
