POSITION SUMMARY The Lead Field Supervisor will play a vital role in overseeing caregiving staff and ensuring residents receive compassionate, safe, and effective care. This full-time leadership position requires flexibility, strong communication skills, and a proactive attitude to collaborate effectively with residents, caregivers, and administrators across multiple locations. A strong background in medication management and oversight is essential, as this role will ensure accuracy, compliance, and staff competency in medication-related tasks. PRIMARY RESPONSIBILITIES • Check on residents daily Create a schedule of facility rounds based on the needs of the residents., • Provide leadership, mentorship, and support to field caregivers., • Assist in resident discharges Remove resident files, medications, and DME from the home., • Observe residents for signs of infection (respiratory and urine), skin tears, wounds, and other changes in condition; report to Administrator., • Be able to follow protocols for calling 911., • Identify new resident care needs and communicate to Administrator; ensure caregivers receive guidance on updated care plans., • Assist with activities of daily living as needed., • Ensure resident vitals are taken daily; monitor weight gain or loss., • Oversee medication management, including auditing resident medications, ensuring correct medications are delivered from the pharmacy, verifying accurate administration, and training caregivers on safe handling and documentation., • Support caregivers in identifying side effects, interactions, or changes in resident condition related to medications., • Collect lab specimens., • Report resident, caregiver, family, and other issues to the Administrator/supervisor., • Engage in social interactions with residents in a friendly and positive demeanor., • Monitor and ensure caregivers are effectively performing their job duties, particularly with medication safety and compliance., • Assist caregivers in keeping the homes, resident rooms, and restrooms clean and organized. QUALIFICATIONS • High school diploma or equivalent required., • Active CNA Certification required., • Criminal background clearance is required., • Minimum two years of experience in an assisted living or healthcare setting, with demonstrated experience in medication management, auditing, and training., • Knowledge of state elderly care regulations and procedures, especially medication-related requirements, is highly valued., • Reliable transportation., • Valid California Drivers License., • First Aid/CPR Certified., • Strong organizational, prioritizing, and analytical skills., • Pre-employment physical and TB Test required per state regulations., • Ability to read, write, and follow oral and written directions.
