Job Summary: Fair Futures Coaches are full-time, trained professionals who are matched with up to 10-15 young people ages 13 and older who have had experiences with the justice system. Coaches meet young people “where they are” and build a trusting relationship with them using an authentic, trauma-informed, and strength-based approach, often in partnership with colleagues at our Non-Secure Detention program. In addition to providing weekly emotional support, Coaches help young people make progress toward their academic, career development, housing/independent living, and other life goals. When a young person is unsure of their goals/interests, Coaches help them explore and expose them to potential opportunities. They then help young people connect to schools, programs, and opportunities that align with their goals/interests, encourage persistence in those settings, and plan/prepare for the next step on their journey. Job Description The Contractor shall provide Contract Facility Manager/Building Manager (CFM) support services to the assigned GSA Facility, Field Office or Service Center. Job Description Summary: The Criminal Court program provides a safe reception center and various services to assist survivors and victims of crime navigate through the Criminal Court. The program provides counseling, crisis intervention, case management services, advocacy, information and referrals and practical assistance to survivors and victims of crime with an overall goal of promoting survivor safety and reducing the impact of victimization. The Coordinator is responsible for providing holistic support services to victims of all crimes. This included counseling and crisis intervention to assess the needs of victims and to help with the development of safety plans to address immediate risks and concerns. This plan may include the provision of in-person or telephone counseling; emergency assistance such as money for transportation; assistance with an Office of Victim Services (OVS) application; providing information and referrals, and follow-up services; and providing assistance with obtaining an order of protection, restitution, and other criminal justice remedies. Job Description IMPACCT Brooklyn is seeking two full-time Social Services Case Managers to support a diverse caseload of vulnerable and at-risk adults. Key Responsibilities Include: • Review all documentation establishing tenants' eligibility for the program and make copies of the files., • Create and maintain tenant files., • Conduct initial intake and assessment of tenants and tenants' families' needs and periodic reassessments., • In collaboration with tenants, prepare initial and periodic revisions of independent living plans, including short-term and long-term tenant goals., • Assist tenants in achieving their goals by identifying and connecting them to community resources and making referrals to appropriate services within and outside IMPACCT Brooklyn., • Work with tenants to break through barriers to their goals and assist them in advocating for themselves and moving toward self-sufficiency., • Conduct group sessions and/or workshops to assist tenants with independent living skills., • Conduct Monthly unit inspections of tenants., • Recommend and implement strategies to persuade tenants to participate more fully in this process., • Monitor tenants' progress toward their goals through regularly scheduled phone calls and/or face-to-face visits, and document it in progress notes., • Participate in case conferences., • Follow up with tenants and referral organizations to confirm tenant contact and progress on referrals., • Provide all required information for weekly/monthly/quarterly/annual reports., • May recommend closing of cases in which tenants have: (a) achieved primary goals and have maintained stability for a period of months; or (b) have not demonstrated a willingness to participate in the process (lost-to-service); or (c) have become ineligible for services (e.g., moved out of building)., • May act as tenant liaison/tenant advocate with outside organizations regarding education, healthcare, housing, legal issues, etc., • May schedule appointments for tenants with referral organizations., • May escort tenants to appointments (educational, medical, social service, etc.), • May assist tenants in completing applications for benefits and entitlements and may process applications on tenants' behalf., • May evaluate tenants' living conditions through home visits., • May prepare marketing materials for the program.
