This includes primary intensive case management/counseling for residents identified as having the most significant psychiatric and/or substance abuse issues. This is currently a hybrid position. RESPONSIBILITIES The Social Worker will manage clinical cases for caseload of residents receiving HRA HASA Supportive Housing Services in addition to seniors, low income working or disabled residents identified has having significant mental health and/or substance abuse issues. Social Worker will assist clients in maintaining medical, emotional and housing stability, utilizing counseling interventions and appropriate referrals and maintain a consistent schedule of face to face client contacts.
