As an RN Case Manager, you'll coordinate compassionate, individualized hospice care by assessing patients, partnering with the interdisciplinary care team, and developing plans that support each patient's needs. The expected base pay range for this position is: New York $32.50 - $36.00 This position will be eligible for a cash bonus, equity rewards and benefits, in accordance with our applicable plans and programs (see more about our compensation and benefits at _).
