Job Description Hospice Lead License Practical Nurse (LPN) Visiting Nurse Health System (Visiting Nurse) is seeking a Hospice Lead LPN providing basic skilled nursing care, patient education, and clinical nursing observations and documentation for hospice patients. This role also assists with facility coordination of care, attending care plan meetings, and providing community education. Hospice care is a special need for those approaching the end of their life journey. In this position... • Provides nursing services as ordered by the physician and under the direction of the RN., • Reports subjective and objective patient findings to the RN., • Visit notes document findings of each home visit. Documents visit findings within 24 hours of the visit., • The content of the visit note reflects the evaluation of the patient’s response to treatment and notifies the primary nurse or clinical supervisor of recommended changes to the care plan., • Actively participates in the weekly team Case Conference., • Provides education to patient & family regarding disease process, self-care techniques, and prevention strategies., • Provides necessary skills to report changes appropriately; documents pertinent information and care rendered to the patient to ensure continuity of care., • Clinical record documentation demonstrates compliance with physician orders for care provided and the frequency of visits., • Complies with the company's policies and procedures., • Documentation demonstrates compliance with Medicare, Medicaid, and other regulatory requirements.
