The Licensed Practical Nurse (LPN) delivers compassionate, high-quality nursing care to clients in their homes or designated facilities, in accordance with the agency’s policies, procedures, and clinical standards. Working under the supervision of a Registered Professional Nurse, the LPN provides direct care, monitors health status, implements physician orders, and supports the overall plan of care. The LPN plays a critical role in promoting client independence, safety, and well-being through effective communication, clinical expertise, and collaboration with the interdisciplinary team. This role requires strong clinical judgment, attention to detail, and a commitment to client-centered care within a home and community-based setting. Hours: Monday-Friday, 9a-5pm Location: Onsite Responsibilities • Provides client care in accordance with the NYS Nurse Practice Act and LPN scope of practice., • Implements medical orders and plans of care under RN supervision., • Follows all agency policies and applicable federal, state, and local regulations., • Observes, obtains, and communicates client condition changes, symptoms, and service needs to the supervising RN., • Educates PCAs on proper medication prompting and client-specific safety procedures., • Performs LPN-permitted assessments and reports findings to the RN; adheres to infection control and PPE standards., • Identifies and reports concerns related to abuse, neglect, or home safety., • Responds appropriately to urgent client needs and promptly notifies the RN., • Evaluates safety and effectiveness of client equipment and supplies., • Supports client self-care and health maintenance education., • Supervises PCAs as assigned to ensure plan of care compliance and accurate documentation., • Completes timely, accurate documentation of all care and client status changes., • Collaborates with the interdisciplinary care team to ensure coordinated, continuous care., • Participates in staff meetings, training, committees, and CQI/PI activities as assigned.Requirements: QUALIFICATIONS • Graduate of an accredited Licensed Practical Nurse (LPN) program., • Currently licensed and registered as a Licensed Practical Nurse in New York State.”, • One to two years of experience providing nursing care in an acute care setting; one year of home care experience preferred., • Strong ability to read and interpret clinical documents, procedural manuals, and safety regulations., • Proficient in basic mathematical functions related to medication dosages and measurements., • Excellent organizational, written, and verbal communication skills., • Demonstrated problem-solving skills with the ability to follow written, oral, or diagrammed instructions., • Demonstrates excellent customer service and interpersonal skills.
