We currently have an exciting career opportunity available to join the HKNC team as a Temporary Part- time Registered Nurse based out of our Residence Facility in Sands Point, NY. The Registered Nurse is responsible for the assessment and treatment of the health care needs (emergent, urgent, and routine) for all participants at HKNC. This requires a well-experienced RN who can work independently, along with in-person and telephone triage/supervision. The RN must be able to communicate their findings to the supervisor and other related healthcare professionals. To provide seamless nursing support, the RN will be called upon episodically to cover nursing shifts in the event of inclement weather or during absences for nursing staff. This position is a temporary leave replacement with an anticipated schedule of 15 hours per week. ESSENTIAL DUTIES AND RESPONSIBILITIES • Familiarizing oneself with all HKNC consumers’ preferred modes of communication, DeafBlind diagnosis, and self-administration status congruent with the New York State Guidelines for the Self-Administration of Medication, • Have a working knowledge of the HKNC Medical Service Department’s policies and procedures, • Have an understanding of HIPAA regulations, • Administer medication to consumers who:, • Are unable to self-administer their medications (prescribed and over-the-counter medications), • Request nursing assistance in the administration of their medications (prescribed and over-the-counter medications), • Monitor the consumer’s response after giving the medication, • Initiates / conducts formal and informal medication education classes with consumers in an effort to foster the consumer’s inter/independence with medication administration including, but not limited to:, • The proper use of a seven-day medication box, • The correct filling of a seven-day medication box, • The correct procedure for ordering prescribed medications, • Provide health education to consumers, including but not limited to:, • Health promotion strategies, • Illness Prevention, • Nutrition, • Exercise, • Participates in the HKNC sponsored vaccination program:, • Provide vaccinations as indicated for HKNC consumers / staff, • Observe individuals for adverse reactions, • Complete NYS or HKNC required documentation, • Tuberculosis Testing:, • Perform health assessments to Identify new symptoms indicating a potentially under / undiagnosed conditions or complications, • Report the findings to all appropriate healthcare provider, including but not limited to:, • Prescriber of Record, • At-Home Healthcare Provider, • Family members as appropriate, • HKNC Director of Medical Services, • Assess / treat each consumer’s unique health problems in a holistic manner recognizing:, • Consumer’s:, • Knowledge deficits, • Importance of participation in the process leading to empowerment, • Need for self-determination, • Execute medical regimens as prescribed by:, • Licensed physicians / MD / DO / OD, • Nurse practitioners, • Physician assistants, • Dentists, • Other licensed providers as authorized, • Contributes as a member of an interdisciplinary health care team to plan and implement the health care needs of consumers, • Position is required to be on-call 24/7 to meet the needs of the residential program. Performs related duties, when necessary, at the discretion of the Director of Medical Service and Administration. Although the hours are assigned during the weekday, it is understood given the needs of the consumers and the inherent nature of the nursing profession, the RN will be called upon to extend their working hours in order to meet the health and safety needs of the consumers. The RN must take the initiative in expediting effective resolution in critical and/or emergency medical situations and relay that information to the nursing supervisor., • Methods of communication used by persons who are DeafBlind at a conversational level must be developed within one year., • Knowledge of deaf and deaf-blind culture must be developed within one year., • Knowledge of MS Office including MS Word, Excel, PowerPoint and Outlook required. Strong report writing skills required. This is an in-person position located out of our group home in Port Washington, NY. We have ONE Full- time shift. Shift: Wednesday-Sunday, 4pm-12am Pay Rate: $22.9440/hour. The Rehabilitation Counselor is responsible for serving as an advisor to each consumer throughout their participation in the program at HKNC. The Rehabilitation Counselor acts as advocate, resource and coach by empowering consumers through supports and challenges to maximize consumer input and responsibility to their training program for achieving their vocational and independent living outcomes, and personal futures plan. Acts as primary liaison with state vocational rehabilitation counselors and consumer for ensuring HKNC services are aligned with the consumer’s abilities, talents, future opportunities and aspirations. This is an in-person position based out of our Training Building in Sands Point, NY. Salary: $40.7204 per hour, $74,111.1862 annualized. This is a temporary leave replacement. Ensures psychological, physical, educational, social-cultural and rehabilitative needs are met as determined necessary and beneficial to the individual to maximize program participation and independence. If necessary seeks consultation with the clinical social worker or team to determine need for other consultative services in order for provide optimal training services for consumer., • Maintains appropriate documentation, i.e., case notes, authorizations, record of student account and aids and devices purchased. Follows policies regarding regular check-in’s for consumer safety, satisfaction and unmet needs., • Supports consumer and team with preparing transition plan and facilitates the coordination of these steps to obtain resources, supports and services in order to successfully achieve long term goals for employment, independent living and community access. Works collaboratively with trans-disciplinary team in developing employment and residential opportunities in home community and other necessary and desired supports. Contacting and consulting with potential employers or residential programs may involve travel., • Participates in Community of Practice (COP) Case Management for shared learning respective of discipline and to gain insight into clinical and vocational rehabilitation counseling issues., • Accompanies and serves as a guide and/or facilitates communication for consumers and professional staff members when assigned. To accomplish this, it may be required to use personal vehicle on occasion. Supports communication access for consumers and staff when necessary in a variety of situations and settings. Examples of settings include but are not limited to: team meetings, airport, shopping, bank errands, phone calls, VP calls, Town Hall Committee meetings, cafeteria, information peer learning groups such as the Emergency Preparedness, Nutrition and Work Solutions groups, recreation activities (i.e. Holiday party). Communication support, which includes providing environmental information, may consist of voice over, sign language (visual/tactual), text/typing, or facilitating the use of augmentative systems (communication cards, devices). The Medical Assistant will be responsible for preparing patients for examinations, assisting with procedures, and managing medical records. On subsequent visits, verifies demographic and billing information to ensure accuracy., • Obtains patient demographic and insurance information, verifies insurance eligibility, and enters accurate information in the electronic health record., • Provides patients with an explanation of all registration forms, obtain patient/guardian signatures on required consents and acknowledgements, and scan all accurately completed registration forms in the electronic health record., • Navigates through insurance portals and accurately interprets coverage information such as the plan coverage description, copayment, primary care provider, etc./, • Schedules patients’ appointments in accordance with criteria established by the health center., • Performs appointment reminder calls and no-show calls., • Performs other clerical duties such as filling, photocopying, and scanning of medical records., • Assists with the preparation of patients prior to seeing practitioners., • Assists the practitioners with patient care in the conduct of clinical examinations and materials preparation, i.e., dressing changes, Pap Smear, etc., • Acts as a translator for patients as needed, • Responsible for taking vital signs, administering tuberculin tests, and performing procedures for specimen collection, routine eye screening, hearing tests, hematocrit, urinalysis, and urine culture, • Performs EKG and simple laboratory procedures, • Distributes lab reports to practitioners for review, • Provides patient counseling regarding procedures (Ex. lab tests), • Assist in the maintenance of inventories and stocking of supplies in examination rooms, nursing, and lab areas, • Maintains logs of required data as needed, • Communicates and coordinates with clinical and school staff to ensure timely patient care., • Acts as the patient’s advocate and liaison for preservation of patient's rights, • Participates in off-site screening projects as needed, • Participates in departmental QA and other patient related committees as needed, • Participates in outreach and community screenings as needed, • Participates in case management and patient surveys in maintaining quality assurance The required skills of medical assisting, phlebotomy, and vital signs monitoring are essential for daily patient interactions and clinical procedures.
