POSITION OVERVIEW The Nurse Practitioner engages in the professional, organizational, and administrative activities to provide quality care to clients based upon their medical and psycho-social needs. The Nurse Practitioner along with the medical director, can develop, implement, and evaluate the medical practice delivery model in accordance with specific goals. The responsibilities of the Nurse Practitioner are leadership, direction and coordination in the provision of medical care and physician services supervised by the medical director and program director. The Nurse Practitioner assumes responsibility for applicable Federal, State and City regulations for the department. KEY ESSENTIAL FUNCTIONS • Participate in planning, development, and organization of medical services provided., • Act as health coordinator to assure the provision of education, risk reduction, counseling and referral services to all patients regarding pregnancy, HIV/AIDS, tuberculosis, hepatitis, sexually transmitted diseases, and all other communicable diseases. Also, training and education will be provided to all staff., • Meet with all scheduled patients efficiently and reviews scheduling with supervisor daily., • Review all medical policies related to patient care., • Conduct physical examinations of patients upon their admission and thereafter on an annual, regular and on an “as-needed-basis.”, • Review the completion and appropriateness of all intake documentation, including patient health history (i.e., other medications and allergies), drug abuse history and previous treatment failures., • Review and approval of initial needs assessment and patient initial treatment plans and reviews., • Participate in preparation for regulatory agency site visits., • Knowledge of Federal, State and City regulatory requirements and ensures all aspects of clinical care conforms to these requirements., • Order, evaluate, adjust and terminate prescription of medication dosages using sound medical, ethical and responsible discretion and meets with the patient to discuss any adjustments in medication dosage., • Review all accident/incidents related to patient care and provides input into actions necessary to resolve identified problems. Implements corrective actions as necessary and appropriate., • Ensure physical participation in the Interdisciplinary Care Planning process., • Prescription and provision of treatment in accordance to patient’s medical and mental condition and only upon their individual needs., • Determine physiological opiate dependency upon admission and patient eligibility for admission to program., • Participate in a weekly and on an “as-needed-basis” case conferencing sessions with MDT members to discuss preventative treatment measure to adverse changes as well as progress in patient’s medical condition., • Assure medical staff compliance with all regulatory and licensure guidelines., • Assure medical staff competence with all applicable policies and procedures of the facility and initiates action in cases of non-compliance., • Participate in the comprehensive care planning, discharge planning and client and staff development by providing ongoing medical training and education to staff. Assume health coordinator role., • Participate medical assessment, consultation, treatment, and referrals that facilitate comprehensive and coordinated patient care., • Participate crisis intervention and consultation on medical/psychiatric emergencies., • Assess client’s need for hospitalization and consultation with client’s Physician to include other care providers the patient is linked with and/or referred to., • Review all patient medication and, if indicated, prescribes medication in consultation with Physician upon patient need., • Provide the Program Director with reports on recommendations concerning the medical services., • Provide general nursing services within scope of license and patient safety goals., • Complete medical records in accordance to organizational policy, • Active and unrestricted New York State Nurse Practitioner license., • Active and unrestricted New York State Registered Nurse license., • Active and unrestricted NPI and DEA registration., • Masters Degree in Nursing/Related field completion of Accredited NP Program required., • Current Infection Control certification., • Current Basic Life Support certification, • Minimum of (3) three years of practical experience required., • Must obtain Mandated reporter (2 hours) training/certificate within thirty (30) days, • Must be board certified by either ANCC or AANP., • Ability to work on a strong team of professionals in a culturally diverse environment., • Ability to multitask and to work and function under pressure. DUTIES & RESPONSIBILITIES: • Conduct street outreach and community presentations to engage and inform drug using individuals about harm reduction interventions., • Conduct harm reduction-oriented individual and group education encounters with drug users to reduce the transmission of blood-borne diseases, other infections, and drug overdose., • Enroll individuals in State-authorized syringe exchange program and provide exchanges that promote the consistent use of sterile syringes and safe collection of used syringes., • Distribute naloxone kits to reverse opioid overdose and provide group and individual trainings on their correct use., • Provide HIV, HCV, and STI testing and facilitate linkage to care, in accordance with standards set by NYS Department of Health/AIDS Institute and CHASI (in-house tester training provided)., • Provide information, referral, navigation, and coordination assistance that links individuals to health and human services responsive to their individually expressed needs and interests. The Doe Fund is also one of New York City’s largest nonprofit developers of permanent affordable and supportive housing, with more than 1,900 units in operation or pre-development. The Doe Fund has an annual operating budget of $80 million and a full-time staff of nearly 500, a significant portion of whom are graduates of our programs. We are seeking a Residence Manager for The Doe Fund's newest permanent supportive housing program for homeless men and women living with HIV/AIDS. The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. Half of the residents will be referred by the New York City HIV/AIDS Services Administration (HASA) and the other half will be apartments for affordable to low-income households. The Residence Manager is responsible for the supervision of the residents and the overall operation of the facility during designated shifts. S/he will work collaboratively with the Clinical team and the Program Director to ensure that resident needs are being met as well as the safety of the facility. Job Description Position Description Enhanced Harm Reduction Specialist The Enhanced Harm Reduction Specialist is responsible for providing an array of harm reduction services to marginalized, street-based populations including people experiencing homelessness, people living with HIV, people who actively use and inject drugs, people with Hepatitis C, people who engage in sex work, and other high-risk populations. This position provides drop-in services, safer use supplies, personal care items and pantry, health education, and assistance with referrals. Overdose prevention education is conducted on a regular basis to all clients with a history of substance use. Services are conducted utilizing a harm reduction approach that supports the client in identifying their own goals around substance use and safer sex. We support clients who are actively using, those who use occasionally, and those who are in recovery. Essential Job Functions Ensure drop-in is set up for the day and that the community room is cleaned up at the end of drop-in hours. Provide direct services to clients in the drop-in, including assistance with intakes, client engagement, referrals, and distribution of needed supplies. Track service delivery for all client services in the drop-in; enter completed services in required databases. Provide occasional coverage for the syringe access program, which may include: • Distribute safer use supplies, • Monitor disposal of syringes being returned into the sharps container and count replacements, • Safeguard inventory, • Adhere to needle stick prevention guidelines, • Provide relevant, accurate, and appropriate risk reduction information via counseling and education, distributing literature and supplies, safer sex supplies, hygiene kits and explaining how supplies may be used to reduce risk;, • Conduct individual and group overdose prevention trainings; these trainings help clients recognize, prevent, and treat an opiate overdose;, • Coordinate referral to patient navigation/medical services. Initiate referrals to additional social support services;, • Provide Risk Reduction counseling and Education relating to HCV prevention and treatment, overdose prevention, substance use and safer sex;, • Provide Hepatitis and health care coordination to appropriate clients;, • Make referrals as needed to low threshold medical services, substance use treatment, mental health services, case management, housing services, food pantry, benefits, emergency needs and other services;, • Maintain all paperwork and submit on-time to Drop-in Program Coordinator and Program Director., • Attend trainings, service planning sessions, and other activities to ensure the provision of the most up-to-date and culturally appropriate services., • Maintain the highest level of professionalism at all times., • Participate in data-driven and clinical supervision, as well as departmental, divisional, and staff meetings., • Experience working with street-based populations, such as people who use drugs, people experiencing homelessness, and sex workers, • Bilingual in Spanish strongly preferred, • Knowledge of harm reduction and the risks associated with injection drug use and sexual health, • Ability to listen to and communicate clearly and respectfully with people who use drugs, community members, peers, volunteers and agency staff, • High level of self-motivation, • Commitment to working as a member of a team, • Able to work with a team with different attitudes and personalities.

We are seeking a Mental Health Counselor for the Crotona Park residence which is The Doe Fund's newest permanent supportive housing program for homeless men and women living with HIV/AIDS. The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. The majority of Crotona Park residents will be referred by the New York City HIV/AIDS Services Administration (HASA) and some by the New York City Housing Authority (NYCHA). The Case Manager/Mental Health Counselor is responsible for providing supportive case management services to the HASA-funded residents. The successful candidate will have strong clinical and communication skills along with an understanding of working with the HIV/AIDS population.
