Position Summary: The Community Health Outreach Worker (CHOW) is a part-time position that assists with the outreach, education and counseling of a specified group of members; consistent with the philosophy, goals and objectives of Amida Care. Assists clients in adhering to their individual disease processes and care management plans. Incumbent may be a Plan member of Amida Care. Responsibilities: · Provide assigned group of members with general information pertaining to the Plan and healthcare access. · Facilitate access to treatment programs. · Provide travel assistance/escort for specific members to scheduled appointments. · Conduct community canvas to locate additional resources and/or possible client locations. · Foster trust in healthcare system and promote treatment adherence. · Act as patient advocate in specific situations and/or discussions with care providers. · Counsel designated clients on basic harm reduction information. · Provide social support and enhancement of self-esteem to assigned clients. · Weekly documentation of outreach efforts in Amida Care member management systems. · Monthly New Member Orientation home visit outreach. Job DescriptionPatient Navigator (MHL)PURPOSE OF POSITION The Patient Navigator provides support, advocacy, and care coordination for individuals accessing mental health and supportive counseling services, with a specialized focus on People Living with HIV/AIDS (PLWHA). The Patient Navigator works closely with clinical and support staff to ensure clients receive comprehensive care, including mental health services, substance use support, housing assistance, and linkage to medical care and social service providers. This role is critical in reducing barriers to care, improving engagement in services, and supporting clients in navigating complex healthcare and social service systems. The Patient Navigator provides culturally responsive, trauma-informed, and person-centered support to empower clients and enhance their overall well-being. RESPONSIBILITIES The major responsibilities of this position include, but are not limited to: Client Engagement & Support • Serve as a primary point of contact for clients seeking mental health and supportive counseling services., • Conduct client intakes, needs assessments, and care plans., • Develop and maintain up-to-date risks assessment, linkage to care, treatment adherence assessments., • Provide education, emotional support, and advocacy to individuals with HIV/AIDS navigating behavioral health care., • Support clients in adhering to their mental health treatment plans and medical care., • Schedule clients for office/home visits to conduct service plan updates, wellness checks, and/or reassessments., • Provide ongoing support to encourage retention of services., • Conduct re-engagement activities via a series of phone calls, written correspondence, and/or in person visits to the client’s place of residence., • Create a safe and professional and welcoming environment for clients of the priority population., • Provides home, hospital, or community site visits and escorts patients to medical and non-medical appointments, as indicated and as approved by the Clinic Supervisor and/or Clinic Director. Job Description Reporting Relationships: The Program Director report to the Chief Operating Officer (COO) Position Summary: This position is responsible for managing a comprehensive residential program of services for victims of domestic violence, including 24-hour resident care, social services, case management, physical plant maintenance, an on-site pre-school day care program, permanent housing assistance and regulatory requirement compliance. Nature and Scope: This position requires keen analysis of complex issues, often involving a large entrenched and service resistant, multi-problem population and creative and constructive responses through the collaboration of several program divisions and public and private agencies to maximize sometimes inadequate or scare resources. An individual family may be faced with all the following problems: inadequate income, no permanent home, single parent, medical neglect, education failure, under parenting, poor nutrition, substance abuse, history of sexual abuse or domestic violence, unemployment, and mental health problems. Managing a comprehensive response to such a situation requires excellent analytical, evaluative and human service management skills. Principal Duties and Responsibilities: Under the general supervision of the COO, Program Directors are expected to perform the following principal duties and responsibilities. · Oversee the physical plant, facilities and grounds to assure proper maintenance, sanitation, safety and code compliance. · Manage the 24-hour resident care, including security, fire safety, crisis intervention, communication and supplies, assuring a structured supervised residential setting for approximately 40 families. · Direct the provision of appropriate child care, recreational, educational and cultural activities in order to provide a respite for parents and children, foster positive child development, enhance self-esteem and create a climate of cooperation and celebration. · Promote a comprehensive plan of social services to establish family stability, address issues of family dysfunction, ensure receipt of appropriate benefits and entitlements and expedite successful relocation to permanent housing. · Plan, develop and implement facility programmatic policies and procedures to ensure uniformity of services within each division, compliance with regulatory and agency requirements and synchronization with other homeless services functions. · Build an interdisciplinary, multi-shift, multi-cultural, professional and para-professional staff. Develop an effective team to ensure coordinated, high quality service provisions and interdisciplinary staff collaboration. · Manage the timely collection, analysis and presentation of clients, programmatic and administrative data to provide timely reports, ensure regulatory compliance, evaluate program performance and determine unmet client needs. · Participate in the development of annual operating budgets through review of financial history, identification of unmet needs and assessment of future trends to ensure the fiscal stability of the program, maximize available resources and provide proper accountability. • Performs other related duties as may be requested. Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. We are seeking a Social Service Supervisor for The Doe Fund's newest permanent supportive housing program for homeless men and women living with HIV/AIDS.
