Role Overview Epicured is seeking Social Care Navigators (Fully Remote) to support our expanding New York Medicaid Section 1115 Waiver initiative. This program is designed to improve health outcomes, expand equitable access to care, and reduce health disparities by helping Medicaid members receive support for health-related social needs (HRSN) and qualifying nutrition-related services. In this role, you will conduct outreach and screening with Medicaid members telephonically to assess eligibility for Epicured services and help navigate members to internal and external resources they qualify for. You will use designated contact center and program platforms to accurately document screening outcomes, referrals, and next steps. This position is ideal for someone who is highly organized, compassionate, tech-savvy, comfortable in a high-volume call environment, and motivated by supporting members with complex needs.
