Job Description PRIMARY FUNCTION/PURPOSE: Supervise the delivery of services provided by Client Care Coordinators (Social Workers) to enhance existing services to homeless families. This includes the completion of a thorough assessment, and the development and completion of a bio-psychosocial for each family upon intake, crisis intervention, counseling, client advocacy, and referrals to government and/or community-based resources. If needed, this individual will ensure that recommendations of achievable goals be implemented on each family’s service plan, leading to permanent housing, and strengthening the overall permanency outcome for clients in shelter. MINIMUM QUALIFICATIONS: • Must be a Licensed Clinical Social Worker (LCSW) in the state of New York upon hire or within three (3) months of hire, • Minimum of 3-5 years of experience in social appropriate referrals and linkages between mental health providers, government agencies, and other related community-based services as needed, • Serve as agency/program liaison between DHS, government that Client Care Coordinators are completing all documentation in a timely manner, • Ensure that Client Care Coordinators assess each client/family for supportive housing, complete supportive housing applications when applicable, and accompany residents to supportive housing interviews when needed, • Ensure that Client Care Coordinators complete and discuss exit summaries with each client, and provide clients/families with referrals and supportive services identified in their new community, • Participate in family meetings with case work and housing teams to discuss planning efforts and progress, • Networking and establishing successful linkages with government and community-based organizations to create resources for both families and staff, • Work in collaboration with the Director More than half of our clients are immigrants and refugees from around the globe. Scattered Site Housing Program provides 270 scattered site-housing units to HIV/AIDS Services Administration (HASA) clients. All the units are leased in the agency’s name. Case management staff focusses on removing barriers to maintain medical care and achieve viral suppression by ensuring that all clients’ basic needs are met and that clients are connected to needed services, such as mental health and substance abuse. Responsibilities: • Learn and integrate a trauma-informed, client-centered approach, using an anti-oppressive lens, into your work with clients. Coach and guide staff in their client practice that reflects these approaches. Participate in the recruitment, hiring and onboarding of new staff., • Provide in-house individual and group psychotherapy, including crisis intervention and emergency assessments;, • Provide supervision and oversight to Case Managers and Peer Navigators, • Advocate for clients in the mental health system, including preparation of psychosocial reports;, • Enhance the clinical component by strengthening staff’s ability to assess mental health needs;, • Conduct weekly one-hour individual supervision with staff to ensure quality of direct clinical services;, • Co-facilitate weekly group clinical case conferencing;, • Participate in weekly supervision;, • Participate in staff process, • Learn and integrate a trauma-informed, client-centered approach, using an anti-oppressive lens, into your work with clients., • Establish linkages with external mental health providers;, • Conduct quarterly review of client charts, • Support the oversight of daily program activities;, • Perform client escorts and visits, • Participate at staff trainings, staff meetings, and staff group process workshops; Qualifications: Three (3) years experience providing clinical supervision and individual therapy — preferably with at-risk youth required. Experience with youth and/or homeless population, utilizing harm reduction philosophy preferred.
