PHACS is a field-based program in which team members visit clients in their homes and accompany clients to appointments. The majority of clients live in the Tenderloin, SOMA, and Mission neighborhoods. Most team members walk and/or access public transportation when completing field work. The ideal candidate for the position has a significant capacity for compassion for others as well as an established practice of self-care and healthy boundaries. ESSENTIAL JOB DUTIES: • Assess client needs via review of reason for referral and medical record, direct client interview, collaboration with existing treatment and care providers.
