Job Description Our client, a leading community-based healthcare provider in the Hudson Valley, is hiring a Nurse Practitioner - Internal Medicine/Primary Care to join their dedicated team in Poughkeepsie, NY. Responsibilities: • Collaborate with physicians and healthcare team members to deliver comprehensive primary care., • Conduct health assessments, physical exams, and preventive screenings based on patient age and history., • Formulate differential diagnoses and interpret diagnostic tests within protocol guidelines., • Develop, execute, and modify treatment plans, including prescribing medications and non-pharmacologic therapies., • Perform hospital rounds, participate in discharge planning, and make appropriate referrals., • Document thoroughly and accurately in EMR, ensuring timely updates and coding., • Engage in quality assurance and continuous improvement activities., • Educate patients and families, promoting health and wellness. Requirements: • Graduate of an accredited Nurse Practitioner Program and licensed to practice in NYS., • 2-5 years of related clinical experience preferred; solid clinical training ground considered., • Strong health assessment and clinical decision-making skills., • Ability to work collaboratively, support a team environment, and adapt under pressure., • Excellent communication skills and commitment to patient confidentiality., • Flexibility to participate in ongoing professional development and quality initiatives.
