Job DescriptionSalary: $17 - $20 Certified Homemaker-Home Health Aide (CHHA) New Jersey Position Summary: The Certified Homemaker-Home Health Aide (CHHA) provides essential personal care and homemaking services to clients in their homes under the supervision and direction of a Registered Nurse (RN). The CHHA supports clients health, safety, and independence by assisting with activities of daily living (ADLs), maintaining a clean and safe environment, and offering compassionate support. Key Duties • Provide personal care and homemaking services essential to the clients health and comfort at home, including shopping, errands, laundry, meal planning and preparation, serving meals, child care, and assisting with ADLs., • Perform all assigned tasks under the supervision and direction of an RN, who is responsible for the care provided., • Never administer medications under any circumstances. Primary Responsibilities • Follow the individualized plan of care to deliver safe, competent, and compassionate care., • Assist clients with personal hygiene and help maintain a healthful, safe home environment., • Plan and prepare nutritious meals and shop for groceries as directed by the nurse., • Assist with ambulation and mobility as ordered by the physician and approved by the nurse., • Support therapy personnel as needed in the rehabilitative process., • Encourage clients to maximize their independence according to the care plan., • Promote mental alertness and engagement through activities of interest., • Provide emotional and psychological support to clients and their families, fostering trust and confidentiality., • Immediately report any changes in the clients condition or home situation to the supervising nurse or aide supervisor., • Perform routine housekeeping tasks related to client safety and comfort as instructed by the nurse., • Complete and submit weekly activity/visit reports promptly for inclusion in the clinical record., • Confirm weekly visit schedules to coordinate with other staff visits as needed., • Collaborate with personnel from other community agencies involved in the clients care as directed by the nurse., • Attend required in-service training and continuing education as mandated by regulation. Job Conditions • Must be able to drive and access clients homes, which may not be wheelchair accessible., • Must have sufficient hearing, vision, and physical dexterity to assess client condition and provide care., • Occasionally required to bend, stoop, reach, and move client weight up to 250 pounds; lift/carry up to 30 pounds., • Must be able to communicate clearly in English, both verbally and in writing. Equipment Operation • Use of blood pressure cuff, thermometer, stethoscope, and hand hygiene materials. Company Information • Has access to all client medical records, which may be discussed with the RN and Director of Nursing as needed. Qualifications • High school diploma or equivalent preferred., • Current, valid CHHA certification in good standing issued by the New Jersey Board of Nursing., • Must be free from health problems that may be injurious to clients, self, or co-workers, and must provide appropriate documentation., • Must understand and respect client rights, including ethics and confidentiality of care., • Current CPR certification required., • Must pass a criminal background check. CHHAs are vital members of the care team, helping clients remain safe, healthy, and independent in their homes while upholding the highest standards of professionalism and compassion.
