Physical Therapist (Full-Time) – Geriatric & Home-Based Rehabilitation OUTREACH Rehabilitation New York & New Jersey Metropolitan Area About OUTREACH Rehabilitation OUTREACH Rehabilitation is a growing multidisciplinary rehabilitation practice providing Physical Therapy, Occupational Therapy, and Speech-Language Pathology treatment throughout the New York and New Jersey metropolitan area. Our mission is to help older adults move better, feel better, and live better through high-quality, patient-centered rehabilitation care delivered directly in the home and community setting. OUTREACH bridges the gap between traditional outpatient rehabilitation and home health by delivering individualized treatment in patients’ homes, senior residences, assisted living communities, and office settings. We are seeking motivated, compassionate, and clinically driven Physical Therapists who are passionate about working with geriatric populations and value autonomy, flexibility, collaboration, and professional growth. Position Summary The Physical Therapist will evaluate and treat older adults in community-based settings, focusing on mobility, balance, fall prevention, functional independence, strength, endurance, and overall quality of life. Therapists work closely with patients, caregivers, physicians, and interdisciplinary rehabilitation team members to develop individualized plans of care that promote safe, functional, and meaningful outcomes. Key Responsibilities • Perform comprehensive physical therapy evaluations and reassessments, • Develop and implement individualized treatment plans for geriatric patients, • Assess home environments for safety concerns and environmental barriers, • Perform balance, gait, and fall risk assessments, • Address musculoskeletal, neurological, cardiopulmonary, and functional impairments, • Provide gait training, transfer training, therapeutic exercise, and neuromuscular re-education, • Educate patients and caregivers on fall prevention, mobility strategies, adaptive equipment, and home exercise programs, • Recommend and train patients in the use of assistive devices and adaptive equipment, • Collaborate with physicians, caregivers, and interdisciplinary rehabilitation team members, • Complete timely and accurate electronic documentation in compliance with regulatory and company standards Qualifications • Doctor of Physical Therapy (DPT) or Master’s Degree in Physical Therapy from an accredited program, • Active New York Physical Therapy License required Preferred Clinical Experience • Geriatric rehabilitation, • Fall prevention and balance training, • Neurological rehabilitation, • Functional mobility training, • Home-based rehabilitation, • Chronic disease management, • Vestibular or cardiopulmonary rehabilitation experience is a plus, • Strong clinical reasoning, communication, and interpersonal skills, • Ability to work independently while remaining collaborative within a team environment, • Home care, home health, outpatient geriatrics, or community-based rehabilitation experience preferred Compensation & Benefits • Competitive salary: $90,000 – $105,000, • Quarterly performance bonus opportunities, • Health insurance benefits, • 401(k) with profit sharing, • Paid Time Off (PTO), paid holidays, and separate paid sick leave, • Continuing education reimbursement, • Company-sponsored commuter benefits, • Comprehensive onboarding, mentorship, and clinical support, • Collaborative and supportive team culture, • Opportunities for professional growth and leadership development Why Join OUTREACH?
