If you don’t have one, visit the NPI website., • 3 Professional Letters of Recommendation, • 3 Professional References (at least 2 supervisors), • Identify places you lived for the past 28 years (Statewide Clearance Registration), • Schedule with IdentoGo for Fingerprinting (This is covered by Callen-Lorde), • Submit your NYS license and registration information, • Complete a Health Assessment (Schedule with your Physician for physical and lab work (titer tests, boosters, and vaccines) ) Social Worker/Therapist ($66,939.60 - $92,500.00 Salary/year) Social Workers are responsible for providing individual, family/couple, and group psychotherapy sessions, supportive counseling, crisis intervention, care coordination, referral services for our Article 28 clinic (short-term therapy), and meet required productivity expectation of 33 scheduled weekly visits. The position will work in collaboration with Callen-Lorde medical staff and consulting psychiatrists, as a member of our multi-disciplinary team. The overall goal of the position is to ensure that patients' psychosocial needs are addressed and met. Work Schedule • Callen-Lorde’s BH Clinic is open Monday through Friday., • This fully onsite position is a Monday - Friday, 35hr work week., • This position also requires the availability to work some evenings.
