CAMBA’s Health Home program is an innovative new model of care management focused on improving the health outcomes for individuals with severe mental illness & other complex chronic illnesses through coordinated networks of medical, psychiatric, behavioral health, social service & housing providers. Staff will use cutting-edge health information technology & a unified plan of care to ensure all clients connect to & maintain primary medical care, prevent hospital stays, &/or reduce the length of hospitalizations. Position: Case Manager Reports To: Supervisor I Location: 2244 Church Ave, Brooklyn, NY 11226/ with hybrid schedule available What The Case Manager Does: • Review all documentation establishing clients' eligibility for program and make file copies (e.g. Medicaid status and confirmation of HIV status)., • Assist clients in completing all CAMBA intake applications and forms., • Create and maintain client files., • Carry a caseload of approximately 50 clients., • Conduct initial intake or assessment of clients and clients' families and/or periodic reassessments., • Conduct case conferences prior to finalizing all assessments / reassessments., • Follow-up with clients and with referral organizations regarding client contact and progress with referral organization., • Work with clients to break through barriers to client goals and to assist clients in advocating for themselves and in moving toward self-sufficiency., • Recommend and implement strategies to persuade clients to participate more fully in this process., • Monitor clients' progress toward their service plans goals via regularly scheduled telephone contact and/or face-to-face home and office visits, and document via service plan outcomes and detailed progress notes (i.e. time of service, type of service, etc.)., • Recommend closing of cases in which clients have: (a) achieved primary goals and have maintained stability for a period of months; or, (b) have not demonstrated a willingness to participate in the process (lost-to-service); or, (c) have become ineligible for services (e.g. moved out of area, change in Medicaid status)., • Provide all required information for weekly/monthly/quarterly/annual reports (e.g. # of clients in managed care programs, # of clients housed, etc.)., • Act as client liaison/client advocate with outside organizations regarding such matters as education, healthcare, housing, legal issues, entitlements, etc., • Escort clients to appointments (educational, medical, social service, etc.)., • Follow-up with clients for a period of time after successful completion of their primary goals to assure client stability., • May plan, coordinate and facilitate social/peer support events, including group facilitation for clients., • May prepare marketing materials for the program., • May input client data and client progress information into automated database., • May reach out and market the program to the community in order to recruit clients., • May prescreen clients over the telephone for eligibility and may schedule intake appointments., • Knowledge about, understanding of, and ability to work closely with, persons with HIV/AIDS and related issues. CAMBA’s Flagstone Family Center provides comprehensive case management services to 160 families who are in a Tier II transitional residence program for homeless families. The facility consists of eight (8) four (4) story walk up apartment buildings on the east and west sides of the street. There will be two main entrances: one for the east side of the street and one for the west. Each building contains 20 apartments of varying sizes, each fully self-contained. We are seeking an Associate Director, Jewish Education to help lead Jewish learning across Mem Global by developing core Jewish content and managing both the full-time Jewish Service Learning Educator and the Jewish Life Specialists contractor team. This role advances Mem Global's vision for meaningful young adult Jewish engagement by equipping Community Builders (including Moishe House residents and MHWOW hosts) with practical tools, frameworks, and support to design their own Jewish learning experiences. The Associate Director mentors educators, develops educational curricula and strategy, and partners closely with the Program Team to design and deliver innovative, high-impact Jewish education. Who You Are and What You'll Do: • You believe deeply in building Jewish community for young adults and are excited to help shape and advance Mem Global's vision for Jewish education., • You bring creativity, initiative, and follow-through, and you care about designing learning experiences that are meaningful, accessible, and impactful., • You will advance our Jewish education strategy by:, • North American Jewish Life Specialists (part-time contractors) and the full-time Jewish Service-Learning Educator., • Leading the creation of Jewish educational content and designing, coordinating, and delivering internal staff education programs and events that engage a broad and diverse audience., • Serving on core teams to develop an integrated Jewish learning strategy across Camp Nai Nai Nai, Camp Nai Cruise, and the Moishe House North American National Conference. Serve as Lead Educator for select Jewish Learning Retreats, helping design and deliver immersive Jewish learning experiences., • Managing the Jewish education budget, including Mem Global's Jewish Incentive Opportunity (JIO) grants for Community Builders., • Act as a thoughtful and credible voice for Mem Global's mission, values, and impact, communicating with integrity, and optimism across internal and external settings. This may include participation in donor meetings and relationship-building efforts focused on Jewish education, public speaking engagements, partner interactions, and field-facing moments., • Manage and oversee Mem Global's Jewish education data collection, analysis, and strategy to inform internal decision-making and strengthen storytelling for both internal and external audiences., • You have a Bachelor's Degree in a related field, as well as a Master's Degree in Jewish Education or Jewish Studies and/or Ordination (Semicha) from an accredited program., • You have at least three years of full-time post-undergraduate professional work experience, preferably in Jewish communal settings and/or working with young adults, with prior leadership experience (e.g. projects, departmental, interdepartmental)., • You have the ability and are excited to travel for learning retreats, immersive experiences, conferences, and external meetings., • You are eligible to work in the U.S. without employer sponsorship. In our U.S. Hub Offices, our team works a hybrid schedule where employees can choose to work remotely 2 days per week and collaborate in-person with coworkers the rest of the week, with Wednesday designated as a shared in-office day for connection and collaboration. Program Description: Funded by the New York City Department of Health and Mental Hygiene (NYC DOHMH), the Harlem Health Advocacy Partners (HHAP) program aims to improve the health of NYC residents by supporting access to health and social services to which they are entitled, providing health coaching to individuals to increase self-efficacy for healthy behavioral changes and disease management, building community capacity to seek and/or create healthy conditions and acceptable services through advocacy to government and other service providers, and providing Group Wellness Activities and high-quality educational workshops. Summary of the Position: The Community Health Worker (CHW) Supervisor oversees a team of five Community Health Workers who will provide health coaching services, community engagement and outreach activities, advocacy campaign events, peer support groups, group wellness activities, and health educational workshops to NYCHA Residents in East & Central Harlem who are diagnosed with poorly controlled asthma, hypertension, and diabetes. The CHW Supervisor will provide direct supervision to Community Health Workers, including quality assurance of individual and group health coaching and wellness promotion activities. The Supervisor works collaboratively with program leadership and peer supervisor to support program consistency, shared learning, and alignment with organizational goals, while maintaining autonomy in the day-to-day supervision and development of their assigned team. The CHW Supervisor will monitor participant transitions with the collective support of the HHAP Team including escalation and participant graduation from the program. In collaboration with HHAP leadership and the peer supervisor, The CHW Supervisor will conduct case reviews, case conferences, and outreach/engagement discussions with all Community Health Workers to provide feedback, share best practices, teach through challenges, and ensure all program participants are making progress towards their health goals. The CHW Supervisor will share responsibility for overall program success and activities, including Health Coaching and Community Engagement efforts. The CHW Supervisor will work with the peer supervisor to monitor Health Coaching activities for the team and collaborate with them on best practices, changes, or concerns. The CHW Supervisor will execute health coaching plans, community outreach plans, monitor community engagement activities, and organize community campaigns with the collective support of the peer supervisors, Program Manager, and the HHAP team. The Supervisor will participate in shared planning of event details, and continuous quality improvement efforts with peer supervisors to strengthen consistency and effectiveness across CHW teams. Specifically, the Community Health Worker Supervisor will: Overall Staff Management and Oversight: • Provide daily supervision, guidance, coaching, and professional development to all reporting CHW staff, as well as support to the entire CHW team, including supervisory coverage for the peer supervisor when they are not available., • Manage HR forms, concerns, and requests for assigned staff., • Monitor staff and team performance, including tracking and monitoring of case documentation, outcomes, and progress towards goals for assigned staff. Complete the annual performance review process., • Work with HHAP Program Manager and peer supervisor to set standard and individualized goals for CHWs., • Assign and monitor caseloads to assess capacity, volume, and productivity., • Review health coaching assessments, workplans, participant action plans, and other case documentation to ensure participants are receiving appropriate coaching and that documentation is accurate and timely; implement strategies in coordination with peer supervisor and Program Manager to address quality assurance needs as appropriate., • Ensure unit is compliant in maintaining updated participant records through electronic case management system (Salesforce)., • Training: Work with the HHAP Program Manager and peer supervisor to implement CHW training, including ensuring compliance with mandatory trainings and identifying resourceful trainings for the CHWs. Support development and maintenance of program training materials, templates and guides, protocols, and CHW resources on SharePoint., • Outreach and Wellness Activities: Collaborate with the HHAP leadership team to establish and maintain an annual outreach and health coaching/wellness activity plan detailing planned activities for the program. Support establishment and coordination of group wellness activities, including partner-run activities., • Scheduling: Act as day-to-day point person for assigned CHW’s schedules and managing the schedule of assigned activities at the developments. This includes assigning CHWs to activities, working with the Program Manager and peer supervisor to maintain the alternate coverage during absences, and covering for peer supervisor and Program Manager in their absence., • Partnership/ Relationship management: In collaboration with HHAP leadership, assign CHWs to NYCHA developments and monitor their progress in scheduling activities at those developments. Maintain and develop partnerships with development contacts and community-based organizations and support CHWs in developing or strengthening relationships with developments., • Advocacy campaigns: In collaboration with HHAP leadership and CHWs, support development and implementation of community organizing campaigns annually.
