Job Description Rate of pay is per visit PURPOSE OF POSITION Under the direction of the supervising Assistant Director of Nursing in Community Care, assesses, plans, implements, and evaluates the patient’s plan of care. Provides direct skilled nursing care and observes and instructs the patient in their place of residence. May coordinate services of other caregivers. AREAS OF RESPONSIBILITIES 1. Job Description In the role of an Advanced Care Provider- Physician Assistant or Nurse Practitioner for the At Home, concierge house calls team you will conduct concierge medical at home visits to Sollis Health patients and treat them accordingly, with the supervision/oversight of an attending Physician. This is a pivotal role as part of a growing organization providing seamless care to both adult and pediatric patients for cases that would usually come into an emergency room or clinic. You will provide a variety of treatments and care during a house call reducing the need for the patient to go to the emergency department and receive care directly in their home. This position will be primarily focused in the New York area. Job Description The RN (Registered Nurse) Case Manager is responsible for providing Rebekah Certified Home Health Agency patients with quality care in a safe and judicious manner. The Case Manager will assist in facilitating and coordinating all services that the Home Health Agency patients require. This role is focused on day-to-day project management rigor in a product-oriented environment: maintaining a clear view of the portfolio, managing concurrent workstreams from prototype through production, running operating cadence, tracking decisions and actions, and producing reliable status communication. Primary Responsibilities · Maintain a consolidated, current view of the ADA portfolio: in-flight products, priorities, statuses, and dependencies · Build and manage lightweight but rigorous project plans across multiple concurrent data science and AI product workstreams · Track progress through the full delivery lifecycle — from POC and pilot to production — ensuring handoffs, sequencing, and blockers are visible and managed · Run standing team and working sessions; track decisions, open items, and follow-ups · Maintain RAID logs, proactively surfacing risks, issues, and dependencies before they become problems · Escalate blockers early, with clear articulation of impact and options · Coordinate across data scientists, engineers, and business stakeholders to keep work moving and aligned · Produce concise, accurate status reporting for senior technical and business stakeholders · Support intake and prioritization processes as the team scales its coverage across lines of business Required Qualifications · 5+ years of experience as a Project Manager on complex, cross-functional initiatives · Proven ability to work in agile or hybrid delivery environments; comfortable with iterative, fast-cycle execution and able to drive progress without relying on waterfall-style SDLC structures · Demonstrated hands-on ownership of standard PM artifacts and operating cadence (project plans, RAID logs, status reports, meeting discipline) · Experience in fast-paced, product- or delivery-oriented environments where priorities shift and multiple efforts run in parallel · Comfort working alongside data scientists, engineers, and other technical practitioners — able to follow technical conversations at a conceptual level, ask good clarifying questions, and translate outputs into clear stakeholder communication — without needing to write or review code · Experience supporting data, analytics, or technology product initiatives · Experience in financial services or similarly regulated industries · Excellent written and verbal communication skills, including the ability to adapt register for both technical teams and senior business stakeholders · Ability to operate independently with minimal day-to-day direction Preferred / Additional Qualifications · Exposure to AI, machine learning, or data science product delivery (e.g., managing a team that built models, automated tools, or AI-powered workflows)

Job Description As a Regional Nurse Manager at Sollis Health, you will provide concierge emergency medical care to Sollis patients in our clinics and have administrative responsibilities overseeing the RNs in your assigned region. You will work to drive Sollis' Mission and Vision forward, creating a patient-centric environment in a concierge medical setting, helping to deliver medical care. Responsibilities: • Work 16 hours clinical and 24 administrative as a Regional Nurse Manager for a 40 hour work week, • Provide medical care for Sollis' patients with the supervision/direction of an attending Physician or Physician Assistant., • Assess and treat patients which includes obtaining clinical samples, performing point-of-care testing, obtaining blood samples, providing general symptom management and recommendations within your scope., • Perform medical procedures that include obtaining IV access, phlebotomy, administering vaccines, performing POC rapid tests, administering IV medications and fluids, and performing EKGs., • Document patient information in our electronic medical record., • Provide direct oversight of Registered Nurses and the care provided to patients in your region., • Responsible for maintaining and tracking medication and vaccination inventory as well as general inventory and quarterly counts, in collaboration with Operations leadership, • Help to develop and implement plans and schedules for fully staffing our nursing shifts, while minimizing the use of overtime and meeting the targeted budget and productivity goals., • Accountable for Medbank management including reporting and quality control., • Order medical supplies, vaccines and medications as needed for patient care and the function of the center. Partner with Operations to order and perform maintenance on medical equipment as needed., • Ensure that all newly hired nurses in your region are fully trained and setup for success, • Work with our Medical Directors and Operations Team to develop and implement process improvement plans, related to nursing functions., • Initiate and participate in the planning, implementation and evaluation of quality assurance and performance improvement activities, in collaboration with a Sollis Nursing Leadership Team and the Regional Medical Director., • Develop monthly shift schedules and entering schedules into Sollis' time tracking system, • Participate in the interviewing and hiring of Nurse candidates for your region, • Collaborate with your HRBP to manage any performance or HR related issues., • Provide formal mid-year and end-of-year feedback direct reports., • Observe, evaluate, coach and mentor nursing staff in your region, holding them accountable to Sollis' standards for quality medical care and great customer service., • Ensure standardization of practice through the implementation of current policies and procedures., • Hold routine staff meetings and huddles focusing on patient care, safety and satisfaction., • Be available by phone or in person for unexpected emergent situations or questions that arise from time to time., • Promote optimal infection control and safety practices.
