Job Description The Unaccompanied Children Program provides long term and transitional residential care and other supportive services to Unaccompanied Children (UC) who are under legal custody of the Federal Government. Working in collaboration with the federal Office of Refugee Resettlement, ORR, our program provides a safe and nurturing environment for youth in a Basic Shelter program located at our licensed residential facilities as well as a Transitional Foster Care program in foster homes in the Bronx, Brooklyn, Queens and Westchester. SPECIFIC DUTIES & RESPONSIBILITIES: • Conduct daily comprehensive review of consumers’ interviews to determine eligible for Entitlements benefits, Medicaid Eligibility, Medicaid Managed Care, Medicare, Private Insurance coverage, Self-Pay, • Case conference difficult cases with the Director for guidance and support to ensure funding for 819 and Part 820 programs are obtained in a timely fashion, • Conduct interviews on new admissions for HRA/DSS/SSI/SSD/Self-Pay, • Expedite consumers with the selection of Managed Care Organization and NY State Market Place at point of admissions and upon Medicaid Eligibility, • Daily Data Entry of Medicaid/Managed Care/Medicare/Private Insurance/MAT in AWARDS and E-Lab, • Navigate Inovalon / eMedNY (Epaces) / STARS / SAAMS / AWARDS / Manage Care System Portals for coverage, • Communicate with consumers regarding insurance coverage, restrictions, and disenrollment (Medicaid/Managed Care, Medicare, and Private Insurance), • Process Medicaid applications and recertifications for consumers through the Department of Social Services, NY State Exchange/Marketplace, • Investigate Medicaid issues related to County/deferral/eligibility/denials/dropped/ recertifications, • Identify consumers with Medical/Pharmacy/Out-Patient restrictions to complete MAP forms for the removal of restrictions, • Process enrollment/disenrollments MAP forms through the Managed Care System (MCS) and to the Medicaid Inspector General OMIG, • Resolve problematic Medicaid Managed Care issues for Part 820 coverage, • Daily update the Entitlement 820 Reports with consumers’ Medicaid (Fee-For-Service) / Expedited /Managed Care/Medicare/Private Insurance/Undocumented, • Complete and submit bi-weekly 820 Medicaid Managed Care Reports, • Submit weekly Medicaid Manage Care Reports to reflect consumers’ coverage for 5 Residential Programs to the Pharmacy, Medical and LabCorp, • High School / associate degree, • Experience with Medicaid Managed Care and Human Service, • Background with DSS/HRA benefits, Recertifications, Medicaid Managed Care, Expediting, enrollment and disenrollment

Job DescriptionSalary: 50,000 - 55,000 You Gotta Believe (YGB) is a New York City-based, national organization specializing in finding permanent families for young adults, teens, and pre-teens in foster care. YGB focuses exclusively on preventing youth from aging out of foster care without family; thus reducing the risk of homelessness, incarceration, and poverty due to a lack of stable connections. Founded in 1995, YGB achieves this mission by collaborating with foster care organizations to recruit, match, and provide indefinite, comprehensive support to adoptive foster parents and youth. The Permanency Specialist position is a full-time role working directly with youth living in congregate care facilities or foster home settings who are in need of a permanent family to prevent them from aging out of care without permanency. The Permanency Specialist uses established, youth engagement and family recruitment methods to develop a trauma-informed plan to help youth achieve permanency by identifying adults in the life of the youth who are willing and able to answer the call and make an unconditional lifetime commitment to parenting. The Permanency Specialist works diligently, collaboratively, and creatively on targeted family recruitment strategies while partnering with YGBs Youth Advocate, Home-finding and Family Supports services teams, foster care agency workers and community stakeholders to ensure excellence in pre and post moral/legal permanency journey of youth. This position is in New York City and will have a hybrid work arrangement. WE OFFER. The position primarily involves traveling in the inner city, standing for two hours or more for workshops and presentations, and computer-based work from a seated position. COMPENSATION.
