Job DescriptionSalary: 40,000-42,000 Job Title: Partial Care Case Manager Department: Partial Care Services Employment Type: Full-Time Location: Ewing Position Summary The Partial Care Case Manager is responsible for providing structured group facilitation, care coordination, and case management services to clients enrolled in the Partial Care Program. This role supports clients progress, treatment engagement, and discharge planning while ensuring all services are delivered in accordance with program requirements, regulatory standards, and organizational policies. Essential Duties & Responsibilities Group Facilitation • Facilitate assigned psychoeducational, therapeutic, and skill-building groups in accordance with the program schedule and treatment plans., • Prepare structured group materials aligned with evidence-based practices and program objectives., • Maintain group safety, engagement, and appropriate boundaries at all times., • Monitor client participation, behavior, and progress during groups., • Provide ongoing case management services to assigned clients, including coordination of care and resource linkage., • Assist clients with appointments, referrals, transportation coordination, and follow-up care as needed., • Collaborate with the multidisciplinary treatment team to support individualized treatment goals., • Participate in treatment planning, case reviews, and discharge planning meetings., • Complete all documentation accurately, thoroughly, and within required timeframes., • Ensure documentation meets organizational, regulatory, and payer requirements., • Maintain client confidentiality and comply with HIPAA and all privacy standards., • Provide therapeutic support, education, and encouragement to promote client stability and progress., • Identify barriers to treatment engagement and collaborate with the treatment team to address concerns., • Communicate effectively with therapists, psychiatrists, admissions, and operations teams., • Participate in staff meetings, trainings, supervision, and quality improvement initiatives. service plans; and relevant professional credential, such as Certified Psychiatric Rehabilitation Practitioner, bachelor's degree in social work, certified rehabilitation counselor, certified alcohol and drug counselor, bachelor's degree in rehabilitation, or bachelor of science in nursing.
