Job DescriptionJob Opportunity: Geriatrician Our organization is in search of a compassionate and committed Geriatrician to enhance and expand an established practice within our medical group focused on Geriatric Medicine, located in Saratoga Springs, NY. This physician will play a pivotal role in realizing our vision for a Center for Successful Aging, ensuring that older adults in the area receive outstanding, coordinated healthcare. The role primarily involves outpatient care, allowing for extended visits that facilitate meaningful interactions with patients and their caregivers, ensuring that all care aligns with individual goals, values, and quality-of-life priorities. Key Responsibilities: • Conduct thorough geriatric assessments and provide consultative care for older adults facing complex medical and social challenges., • Work closely with the hospital's inpatient consultative geriatric service to ensure seamless transitions and continuity of care across various settings., • Serve in a consultative capacity with community primary care providers, offering expertise in areas such as geriatric syndromes, polypharmacy management, functional assessments, dementia care, and advance care planning., • Deliver care primarily in an outpatient clinic, with occasional inpatient coverage during vacations., • Allocate extended time for visits to focus on advance care planning, understanding care goals, and facilitating shared decision-making., • Collaborate with other members of the Geriatric team, including advanced practice providers, case managers, social workers, and interdisciplinary care teams to enhance patient outcomes through value-based, patient-centered care., • Actively engage in the Geriatric Medicine Fellowship Program, providing mentorship and teaching to fellows., • Explore opportunities to teach Family Medicine residents based on personal interest., • Contribute significantly to the development and growth of the hospital's Center for Successful Aging initiative.Qualifications:, • MD or DO with board certification or eligibility in Geriatric Medicine (Internal Medicine or Family Medicine trained)., • Strong clinical expertise in geriatric medicine, including diagnosis, management, and care planning for older adults., • Dedication to value-based, patient-centered care models with a consultative approach., • Exceptional communication and interpersonal skills with patients, families, and healthcare professionals., • Interest in program development and innovation within Geriatric Medicine.Strengths of the Role:, • Extended visit times that promote meaningful engagement with patients and caregivers., • Outpatient focus with the flexibility to provide care in clinic, home, and assisted living environments., • Strong collaboration with inpatient consultative geriatrics, ensuring excellent continuity of care., • Integral involvement in the establishment of the hospital's Center for Successful Aging, with opportunities for leadership and innovation., • Teaching geriatrics fellows as part of an accredited fellowship program, with optional residency teaching., • A supportive, team-oriented atmosphere within a well-respected multispecialty hospital-based medical group.About Us: Our hospital is a 171-bed independent community facility, recognized as a leading healthcare provider in the region, known for delivering high-quality, personalized, and compassionate patient care.
