Outreach Ambassador JOB SUMMARY Under the direction of the Director of Outreach and Engagement (OED), the BoldAge PACE Ambassador is responsible for driving program growth through strategic outreach, relationship development, and high-quality prospect engagement. This position serves as the front line of the Outreach & Engagement team, identifying, qualifying, and converting prospective participants into enrollments while educating the community about the PACE model of care. Ambassadors represent BoldAge with professionalism and purpose, embodying our values of People First, Seek to Understand, Exceed Expectations, Do the Right Thing, and Be Bold. They build trust with referral partners, healthcare providers, and community organizations, positioning BoldAge PACE as the premier solution for older adults who wish to remain safely and independently in their homes. Develops and cultivates the PACE organization’s network of business, healthcare, and community relationships by: • Developing a deliberate and organized approach to identifying and engaging with referral partners within the community, • Actively participating in community groups, • Presenting at community events about PACE and care of older adults, • Cultivating and maintaining positive and constructive relationships with healthcare providers, senior service providers and other referral sources, • Establishing positive relationships with potential participants, their families, and caregivers, • Responding timely to all inquiries and commitments, • Evaluating and learning about the needs of the community, especially older adults, and their families, and incorporating learned information into community outreach, • Facilitating the assessment and eligibility determination processes by identifying quality referrals and collaborating with other enrollment staff and the interdisciplinary team. ESSESNTIAL DUTIES AND RESPONSIBILITIES: • Develop and execute a strategic outreach plan aligned with the department’s annual and site-specific growth goals., • Identify, qualify, and cultivate high-quality referrals from healthcare providers, community organizations, and senior service networks., • Conduct consultative conversations with prospects and caregivers to understand their needs and align those needs to PACE services., • Maintain timely and accurate documentation of all outreaches, referrals, and prospect activities in the CRM., • Build and maintain strong relationships with referral partners, ensuring regular communication and follow-up., • Represent BoldAge PACE at community events, educational presentations, and networking opportunities to increase awareness of the PACE model., • Use knowledge of service area demographics, market data, and referral trends to guide outreach priorities and strategies., • Collaborate with Engagement Navigators, Benefits Specialists, Level of Care Assessors, and site Interdisciplinary Team (IDT) members to ensure seamless transitions from referral to enrollment., • Meet or exceed individual goals for referral generation, enrollments, and outreach activities., • Apply a person-centered, inclusive approach to all outreach interactions, optimizing every encounter with prospective participants and referral partners., • Participate in team meetings, continuing education, and professional development to maintain knowledge of PACE principles, policies, and compliance standards., • Maintain confidentiality, uphold all OSHA and infection control protocols, and contribute to a safe and positive work environment., • Support organizational Quality Improvement initiatives and represent BoldAge with integrity in all internal and external interactions., • Perform other duties as assigned to support organizational goals and PACE operations. EXPERIENCE AND EDUCATION • High school diploma required; Bachelor’s degree in Marketing, Communications, Business, Healthcare Administration, or Human Services preferred., • Minimum of 3 years in community outreach, relationship-based sales, or business development — ideally in healthcare, aging services, or nonprofit settings., • Proven ability to meet or exceed sales, referral, or outreach goals., • Knowledge of PACE, Medicare/Medicaid eligibility, and local senior service systems preferred., • Proficiency in CRM systems and Microsoft Office Suite., • Strong written, verbal, and public speaking skills., • 1 year experience working with the frail and elderly population, if this is not present training will be provided. PRE-EMPLOYMENT REQUIREMENTS: • Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance., • Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact., • Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.
