Job Description POSITION SUMMARY The Health Educator is responsible for the coordination of health education workshops and group nutritional education classes, the review of client social determinant of health assessments, food vouchers, referrals, program eligibility documents, and the tracking and documenting clients’ group attendance and progress in the program.
