Job Description Position Description The Patient Navigator is responsible for providing outreach, engagement, and patient navigation services to individuals who are contacted through the SAMHSA program. They will conduct screenings, service orientation and refer individuals internally and externally to appropriate community resources for needed services. They will also enroll eligible individuals into care by navigating and enrolling them into the SAMHSA program and additional services as needed, linking them to our community-based HIV/HCV testing, as well as medical care at Harlem United Health Services as indicated. They will conduct outreach (phone, field) to clients who have been lost to care/follow up and establish ongoing relationships to ensure successful connection to quality medical and social services. Conduct preliminary interviews/screening for program to assess client’s immediate needs, including medical, insurance, case management, mental health, housing, treatment adherence, and other supportive services., • Provide referral services to at least 15 clients per month. Referral services involves the implementation of a brief service plan leading to successful connection to harm reduction and substance use services, including substance use treatment, medication assisted treatment, detox programs, outpatient services, groups, or other support services. Verification of referral services is required of all clients. Completion of service verification is required for all clients., • Complete all necessary programmatic paperwork including assessments, programmatic intakes and reassessments, and service plans., • Ensure the maintenance of appropriate, accurate, and clear documentation of all outreach, education, and client activities, including chart documentation and progress notes., • Complete data entry for client services into required online systems as indicated by funder and by internal Harlem United documentation standards., • Through continuous quality improvement activities, ensure that all clients referred to medical services are connected to care and attend their first medical visit. Work with the Warehouse team to ensure that product is stored in correct areas and that all weights and item codes associated with inbound loads are correct., • Ensure product is shipped before expiration, working to minimize waste and increase inventory throughput, with an emphasis on perishables and produce., • Conduct daily warehouse inspections with the specific goal of overseeing expiration dates, food safety (cold chain and temperature zones) and quality monitoring., • Accurately manage inventory being stored in FRC by third parties. Work with appropriate team members to ensure that product is properly tracked., • Analyze transaction activity in collaboration with warehouse leadership and mitigate bottlenecks. Systems and Documentation: • Support efforts to streamline current WMS and IMS systems alongside the Manager, Procurement & Inventory Controls., • Assist the Procurement & Inventory Controls Manager to create and maintain, as appropriate, detailed formal procedures which document every step necessary to perform inventory transactions (SOP creation and maintenance). As needed, deliver dynamic inventory data and reporting for analysis., • Manage the accuracy and compliance of inventory policies and procedures, with a goal of consistent adherence, in support of internal and external audit requirements. Cycle Counting and Audits: • Implement and lead physical end-of-year (EOY) inventory counts, ensure that other areas of SC team are familiar with process and goals., • Oversee City Harvest’s formal cycle counting program. Elements of the program include:, • Leading the cycle counting process and auditing the results., • Analysis and resolution of inventory discrepancies discovered., • Root cause analysis, problem resolution, retraining as necessary, and reporting of results., • Support audits and inspections by any and all regulatory agencies., • Lead special projects as appropriate., • Maintain AIB standards and actively participate in preparing for audits., • Stay up to date on any internal or external trainings and best practices. Repacks and Receipting: • Oversee inventory for City Harvest repacks. Ensure that all repacks have appropriate product types and quantities., • Manage data collection for repacks so that product can be reconciled in a timely manner., • Reconcile all repacks in a timely and accurate manner to ensure that repacked product does not stagnate at FRC., • Receipt all City Harvest pounds to various food providers (Feeding America, Feed the children, Etc.) Job Specifications: • Knowledge:, • Ability to multi-task under tight deadlines in a high paced environment, • Ability to think/work independently, using good judgment when situations arise for which there is no established protocol, • Ability to work in a team environment and communicate necessary information to team members in a remote work setting, • Timely follow up and attention to detail, • Food Safety standards especially as related to fresh produce handling and supply chain logistics are a plus, • Technical Skills:, • Directly-related experience in a food warehouse with different temperature zones- cooler, freezer, dry, • Hands-on ERP and WMS experience, with a focus in inventory management, • Strong communication and organizational capability., • Demonstrated, successful project management experience is a plus, • Proven teamwork and ability to work with different groups with competing interests Required Experience: • 1-2 years of relevant inventory-related experience, preferably in cold-chain or food service industry, • Basic knowledge of concepts, practices and procedures related to Supply Chain, • Detailed oriented, highly organized and enjoy coordinating activities, • Excellent time management skills with the ability to prioritize and make time-bound decisions, • The ideal candidate will be determined, outgoing, positive, articulate and diplomatic Education: • Bachelor’s Degree preferred, or relevant work experience Working Conditions: • Combination of office and warehouse environment, with time spent auditing and investigating inventory in all warehouse zones, including freezer., • 6:00am start time, • Ability to work on weekends once a month, • Working in powered equipment at different heights, • Ability to meet physical requirements and lift over 30 lbs., • The Inventory Controls Manager will work at our facility in Sunset Park, Brooklyn.
