Evaluate, interpret, monitor, and document the nutritional status and nutritional needs of patients using established standards of care and practice guidelines. Identify and provide age-specific nutrition counseling to meet the cultural needs of the patients and their families. Develop, review, update and implement educational materials to meet the needs of patients and professionals. Write orders for diet, supplement, enteral/parenteral nutrition, and laboratory data per clinic policy. Function as an integral member of the clinical interdisciplinary team. Maintain registered dietitian (RD) status including continuing professional education per CA standards. Precept dietetic interns. Travel, and after-hours and/or weekends, will be required for the purpose of attending community events. Schedule: 4 days per week, 10 hours per day, Monday - Thursday, 7:00 am - 5:30pm | Location: San Bernardino, CA ESSENTIAL FUNCTIONS AND DELIVERABLES • Provide nutrition services and medical nutrition therapy integrated with the patient's medical goals., • Evaluate, interpret, monitor, and document the nutritional status and nutritional needs of patients using established standards of care and practice guidelines., • Identify and provide age-specific nutrition counseling to meet the cultural needs of the patients and their families., • Develop, review, update and implement educational materials to meet the needs of patients and professionals., • Write orders for diet, supplement, enteral/parenteral nutrition and laboratory data per clinic policy. Function as an integral member of the clinical interdisciplinary team., • Coordinate all phases of nutrition care including nutrition assessment, care planning, monitoring, education and counseling., • Create nutritional programs based on the health needs of patients., • Consult with Care Management teams and physicians on complex patients., • Meets directly with patients to identify educational needs; provide individual instruction, utilize informational handouts/booklets, or refer to community resources as needed., • Identifies ways to improve care of patients and make recommendations to the Medical Director and P&T Committee; Participates in various patient program committees., • Performs research and recommends ways to expand and improve nutrition programs., • Participates in quality improvement of programs in conjunction with other services offered., • Works with other departments on HRSA and FQHC reporting and auditing standards., • Attend all CRC events, community, weekend events., • Education: Bachelor's degree in Nutrition & Dietetics, or similar study from an Institution accredited by the Council for Education in Nutrition and Dietetic (ACEND) required. Master's degree preferred. Must have completed the 1200 hours of internship., • Licensure/Certification: Current license issued by The Commission on Dietetic Registration (CDR) required. Current CPR/BLS certification (must be American Heart Association or Red Cross accredited program), valid California driver's license, and auto insurance. As a requirement of this position, you must receive EPIC certification for the module you have been hired into., • Experience: Minimum of two (2) years of experience in an outpatient clinic preferred. Community Clinic experience preferred., • Essential Technical/Motor Skills: Computer and software program knowledge necessary to the position is required, including Microsoft Word, Excel, and other various programs. Manual dexterity to operate office equipment such as a telephone, computer, keyboard, printer, copier, fax machine, as well as other equipment necessary to the position., • Interpersonal Skills: Possess excellent communication and listening skills with the ability to convey information and interact and communicate positively, effectively, and professionally verbally and in writing with all levels of staff, providers, residents, and patients. Must work well in a team environment and perform as a flexible team player to accomplish goals and objectives with various levels of staff and management. Must display integrity and dependability. Ability to gather, analyze, and interpret complex data, create accurate meaningful information, define problems, make strong recommendations and draw valid conclusions for data reporting and decision support., • Essential Mental Abilities: Must be flexible to perform a variety of tasks.
