Job Description Job Title/Position: Registered Nurse Case Manager Classification: Non-Exempt Reports To: Director of Nursing and Executive Director/Administrator Job Description Summary The Registered Nurse Case Manager is responsible for coordinating and delivering hospice care to patients and families in their homes and community settings. This role includes conducting hospice admissions, comprehensive assessment, care planning, case management, patient and family education, and collaboration with the interdisciplinary hospice team to ensure high-quality, compassionate, and compliant end-of-life care. Essential Job Functions/Responsibilities • Conduct comprehensive assessments of patients and families to identify hospice needs, including physical assessments, symptom evaluation, and review of medical history., • Perform hospice admissions and start-of-care visits, including comprehensive assessments, verification of hospice eligibility, patient and family education, and completion of all required admission documentation in compliance with regulatory standards., • Develop, implement, and regularly update individualized plans of care based on nursing assessments, patient goals, and physician orders., • Serve as the primary Case Manager for assigned patients, coordinating services and ensuring continuity of care across the interdisciplinary team., • Administer medications and treatments as prescribed and evaluate patient response to interventions., • Provide ongoing pain and symptom management and adjust interventions in collaboration with the physician and hospice team., • Provide education to patients and families regarding disease progression, medications, symptom management, safety, and care techniques., • Maintain accurate, timely, and thorough clinical documentation that reflects patient status, care provided, and outcomes., • Communicate changes in patient condition promptly to physicians and interdisciplinary team members., • Promote and maintain a safe care environment for patients and families., • Provide emotional support and counseling to patients and caregivers throughout the hospice journey., • Collaborate with social workers, chaplains, hospice aides, and other interdisciplinary team members to address patient and family needs., • Arrange for necessary medical equipment, supplies, and services to support patient care., • Supervise and delegate appropriate tasks to hospice aides and ancillary personnel., • Participate in on-call rotation in accordance with organizational policy., • Demonstrate the ability to work independently while effectively managing assigned caseload responsibilities., • Participate in quality assessment and performance improvement activities., • Engage in continuing education, in-service training, and ongoing professional development., • Perform other job-related duties as assigned to meet patient care and organizational needs. Position Qualifications • Graduate of an accredited nursing program., • Current, unrestricted Registered Nurse (RN) license in good standing., • Minimum of two (2) years of nursing experience; hospice, home health, or community health experience preferred., • Current CPR certification., • Valid driver’s license and access to an insured, reliable vehicle., • Strong assessment, critical-thinking, communication, and organizational skills., • Ability to work independently, prioritize workload, and manage time effectively., • Knowledge of hospice philosophy, interdisciplinary care models, and end-of-life practices. Working Conditions & Expectations • Works primarily in patient homes, assisted living facilities, skilled nursing facilities, and community-based settings, with occasional in-office responsibilities., • Requires reliable transportation, a valid driver’s license, and active auto insurance., • Requires flexibility in scheduling to meet patient needs, including urgent visits or changes in condition., • Participates in on-call rotation and after-hours support as assigned., • Works in a fast-paced, emotionally sensitive environment requiring professionalism, compassion, and adaptability., • Expected to uphold hospice philosophy, professional boundaries, and patient confidentiality at all times.
