We are an Enhanced Assisted Living that is looking for a qualified Licensed Practical Nurse/Case Manager to join our beautiful community. CANDIDATE SHOULD HAVE AN ASSOCIATE DEGREE FROM AN ACCREDITED COLLEGE OR UNIVERSITY WITH MAJOR WORK IN HUMAN RESOURCES OR SERVICE DELIVERY, AND THREE YEARS OF FULL TIME EXPERIENCE IN THE PROVISION OF SERVICES TO A DEPENDENT ADULT POPULATION. In our upscale senior living community Wellness/Resident Services Center, an LPN will supervise, guide, and develop a group of HHAs, resident care aides, and medication technicians, along with playing a crucial role in all aspects of care management, including care planning, coordination, and health assessments ANY SENIOR LIVING EXPERIENCE PREFERRED ADVANCEMENT OPPORTUNITIES WITH A GROWING HEALTHCARE COMPANY!! BE PART OF THE ADMINISTRATIVE TEAM AT A ENHANCED ASSISTED LIVING FACILITY!! Schedule: Tuesday-Saturday Responsibilities include: • Conduct initial intake calls as well as scheduled and urgent patient outreach based on individual patient’s needs and risk levels to review and update the care plan, monitor progress, ensure needs are met, and identify new areas of concern., • Overseeing care to our residents including a dedicated dementia unit, • Support patients during care transitions, including outreach and assessment during and post hospitalization to ensure discharge needs are addressed, to facilitate provider follow-up, and to perform medication reconciliation., • Administer injections, prescribed medications, enemas etc., clean and dress wounds and assist with other basic medical care tasks, • Schedule and supervise wellness staff, • Assist in the admissions and discharge process; maintaining accurate case notes ,keep accurate client records, logging Resident interactions, updating resident charts including filing., • ISP process, Incident Reporting, Falls Management Program as well as coordinating outside services such as Rehab, Home Care or Psychological Services., • A current NY LPN License, • Willing to train no experience needed., • A successful background in nursing case work or health advocacy role, • Ideal Candidate will have some experience with an adult/geriatric population in a social work or discharge planning position preferred, • Ability to understand the needs of a senior and Alzheimer's/Dementia based population, • Top notch communication skills, both written and verbal, • Strong attention to detail, exceptional follow through and time management skills, • Highly organized, ability to multi-task and thrive in and extremely fast paced, ever changing environment, • Exceptional interpersonal and relationship building skills, • Ability to converse will all with a diverse audience in a clear and concise manner, • Demonstrated leadership skills
