Job DescriptionJob Title: Registered NurseJob Description This Registered Nurse role focuses on delivering high-quality primary and preventive care in a community health setting. The nurse collaborates closely with physicians to develop evidence-based treatment plans, support safe and effective medication use, and provide patient-centered education for the self-management of acute and chronic conditions. The position plays a key role in serving underserved and vulnerable populations while promoting health, wellness, and long-term continuity of care. Responsibilities • Collaborate with physicians to prescribe medication dosages, routes, and frequencies based on patient characteristics such as age and gender, in accordance with clinical orders and established protocols., • Support physicians in prescribing medications based on efficacy, safety, and cost, ensuring adherence to legal and regulatory requirements., • Develop treatment plans that are grounded in scientific rationale, standards of care, and professional practice guidelines at the direction of the physician., • Counsel patients on prescribed drug regimens, including possible side effects and interactions with food supplements, over-the-counter medications, and herbal remedies, as directed by the physician., • Recommend and reinforce interventions to modify behaviors associated with health risks, following physician guidance and care plans., • Educate patients and families on self-management strategies for acute and chronic illnesses, tailoring instructions to individual circumstances, literacy levels, and cultural backgrounds., • Triage patient phone calls and walk-in visits, assess urgency and clinical needs, and route or escalate care appropriately., • Apply nursing standards and competencies to deliver safe, effective, and compassionate care in a primary care and community health environment., • Provide case management and managed care support, helping patients navigate services, follow-up appointments, and continuity of care., • Utilize eClinicalWorks or similar electronic health record systems to document assessments, interventions, patient education, and care plans accurately and timely., • Offer supportive care and engagement for individuals with substance use challenges, using nonjudgmental, trauma-informed approaches., • Exercise supervisory responsibilities as needed, providing guidance and support to other clinical or support staff in alignment with nursing standards., • Communicate clearly and professionally in English, both orally and in writing, with patients, families, and interdisciplinary team members., • Contribute to a culture of growth, education, and promotion from within by participating in ongoing training, quality improvement initiatives, and professional development activities.Essential Skills, • Active Registered Nurse (RN) license with eligibility to practice in New York State., • Bachelor of Science in Nursing (BSN)., • 14 years of nursing experience, ideally in a primary care setting., • Current CPR certification., • Demonstrated competence in nursing standards, clinical assessments, and core nursing skills., • Ability to communicate effectively in English, both orally and in writing., • Experience with case management and managed care in an outpatient or community health environment., • Proficiency using eClinicalWorks or similar electronic health record systems., • Ability to triage patient calls and walk-in visits, prioritizing care based on clinical urgency., • Experience working with individuals affected by substance use or substance abuse., • Strong commitment to public and community health and service to underserved populations.Additional Skills & Qualifications, • Experience in a Federally Qualified Health Center or similar community-based healthcare organization., • Supervisory experience in a clinical setting, with the ability to support and guide other staff., • Ability to work effectively with individuals from diverse ethnic, cultural, and socioeconomic backgrounds., • Comfort providing behavioral health-related education and support as part of integrated primary care., • Interest in long-term professional growth, education, and advancement within the organization., • Alignment with mission-driven work focused on comprehensive primary, preventive, and behavioral health services.Work Environment This role is based in a community health center environment serving diverse neighborhoods in New York, with openings in Yonkers, New Rochelle, White Plains, and Rockland County (including Haverstraw, Nyack, and Spring Valley). The typical schedule is Monday through Friday from 9:00 a.m. to 5:00 p.m., with 12 late evenings per week until 7:00 p.m., on days when the start time is adjusted later. The setting emphasizes comprehensive primary, preventive, and behavioral health services for patients regardless of insurance status or ability to pay, with a particular focus on underserved and vulnerable populations. Staff use electronic health record systems such as eClinicalWorks and work collaboratively within an interdisciplinary team. Job Type & Location This is a Contract to Hire position based out of White Plains, NY. Pay and Benefits The pay range for this position is $45.00 - $52.00/hr. Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
