POSITION SUMMARY: A Home Health Aide is responsible for assisting the residents in their activities of daily living, and in maintaining the social and physical environment for their comfort, safety and well-being in accordance with Department of Health regulations, community standards, and also as directed by a supervisor. The ideal candidate must possess strong interpersonal skills in order to deal with the physical and social needs of our resident population. The ability to communicate calmly, patiently, and effectively with residents, families, visitors is a must. This is a Full-time hourly position in our Gerald Levine Memory Care unit from 11pm-7am. You will work a 5 day on/2day off rotating schedule in which ever other weekend is a must. PRINCIPAL RESPONSIBILITIES: • Work under the supervision of the Registered Nurse/LPN in accordance with NYS DOH Regulations, and community standards., • Provide residents with assistance in Activities of Daily Living (ADL’s), including but not limited to bathing, dressing, grooming, toileting, changing bed linens, laundry, incontinence care as needed., • Follow the resident’s ISP (Individualized Service Plan/Care Plan), and the HHA Care Plan, and complete daily task sheets/assignment., • Completing required documentation accurately, and timely., • (If trained as med tech-HHA), Assist with and properly document medications, following accepted practices, and in accordance with DOH and Community standards., • (If trained as med tech-HHA), order meds when running low, receiving/signing for medications from pharmacy, notify LPN/RN is refills needed, or any questions, issues, or concerns with medications., • (If trained as med tech-HHA), document on 24 hour report, and shift change report., • Observe assigned, and all residents, and report any changes, or concerns regarding resident, or care to LPN/RN/Supervisor., • Respond to call signals/pull chords/requests for help, call for emergency services (911) if needed. Report incident on designated forms, and complete appropriate paperwork., • Assist with dining services as necessary., • Encourage residents to attend activities, assist to get to activity as needed., • Attend required (12 hour minimum) in-servicing, and as directed by supervisor/managers., • Collect, wash, dry, fold, distribute laundry/linens., • Follow, participate in all established fire/safety components, including in participating in monthly drills, and fire safety training., • Report and accidents/incidents immediately., • Meet/greet all residents, families, visitors, departments in a friendly, respectful, helpful, courteous manner with resident/family satisfaction always being a priority., • Provide all/only services within your Home Health Aide Certification/Scope of Tasks., • Frequent contact with residents, families, visitors, and other departments., • Flexibility with the work schedule, including day/evening/night hours to meet the residents needs and may be required to do mandatory overtime., • Ability to manage multiple interruptions, and ability to make independent decisions when conditions warrant, and to demonstrate good judgment in areas of responsibility., • Represent the Community in such a way to exhibit maximum professionalism reflecting accuracy, attention to detail, and overall competency., • All other duties as assigned Minimum Requirements Mental/Visual/Physical Demands: • Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions., • Must be mentally alert and the ability to effectively plan and organize assignments in the most efficient manner., • Must be flexible, adaptable, and able to function in stressful situations., • Must possess sight/hearing senses, or use prosthetics that will enable these senses to function adequately so that the requirements of the position can be fully met., • Physical effort includes walking, bending, standing, squatting, climbing stairs, walking on uneven surfaces, reaching above shoulders, and lifting up to 50 lbs., • Must have manual dexterity requiring the use of repetitive action, forearm rotational movement, writing simple grasping, fine manipulating, power grip, pushing, lifting, pulling. Emergency’s situations may require crawling, climbing stairs. Moderate to heavy risk of exposure to blood-borne pathogens., • Extended periods of walking and standing are required Education/Experience: • HS Diploma or GED required., • Current certification (certificate provided of completion of a NYSDOH approved HHA Program), • Experience as Home Health Aide working with Alzheimer’s/elderly population preferred.
