Work with the team to ensure that referrals and applications are made as and when necessary and within the set time frames e.g. Safeguarding, Deprivation of Liberty ? Contribute to the process of assessment of referrals as and when required ? Work with the care team to coordinate admissions of suitable Children and Young People to the Home ? Ensure that all the relevant personal/pathway/placement plans for Children and Young People are in place, accurate, up to date and are adhered to by staff ?
