We have an exciting opportunity to join Our SinglePoint of Access Team. Our SinglePoint of Access service is a newly developed team at the hospice and this is an exciting opportunity to be a part of its ongoing growth and development. As a key member of the team you will play a vital role in ensuring that patients receive the right care at the right time. This service acts as the central point for external referrals into the hospice assessing urgency and need to support seamless patient care. Working closely with the wider clinical teams we strive to maintain patient flow and deliver a responsive high-quality service. It is a growing team and you will have the opportunity to contribute to ongoing service developments and ideas. About the Role Hours: 37.5 per week Monday to Friday 9am until 5pm You will work alongside a clinical assessor and care co-ordinators to prioritise and manage referrals liaising directly with patients families and referrers to ensure timely and appropriate support. You may respond to advice calls offering professional guidance to patients carers and healthcare professionals while signposting them to relevant services as needed and appropriate. Key Responsibilities: • Assess referrals to determine urgency and priority ensuring timely responses for patients and referrers., • Provide advice and signposting support to referrers patients and families., • Collaborate with the Single Point of Access and Responsive Care team lead in shaping the SinglePoint assessment process and policy., • Contribute to the ongoing development of the service through a quality improvement approach., • Work closely with patient-facing teams to streamline care pathways and ensure efficient service delivery., • Develop strong working relationships with primary care professionals and other referrers to enhance referral processes and feedback mechanisms. About You We are looking for a highly motivated and compassionate Registered Nurse (RN) or Allied Health Professional (AHP) who thrives in a dynamic environment and can confidently manage and reprioritise their workload. You will be proactive empathetic and work collaboratively within a multidisciplinary team to deliver the highest standard of care. With a keen eye for detail and excellent communication skills you will be able to assess and respond to individual patient needs effectively. Strong organisational skills an approachable nature and the ability to engage well with colleagues and external partners are essential for this role. Key Skills and Experience: • Registered Nurse (RN) or Allied Health Professional (AHP) with relevant professional registration although we will consider other relevant skills and experience, • Educated to degree level or equivalent experience., • Experience in palliative and end-of-life care supporting patients and families through complex health needs., • Proven ability to liaise and collaborate with multidisciplinary and multi-agency teams., • Strong facilitation and negotiation skills., • Experience in handling challenging and complex situations with professionalism and care., • Ability to work under pressure and manage priorities within set timescales., • Excellent organisational skills with the ability to adapt and reprioritise workload as needed., • A flexible and adaptable approach to meet changing service demands and patient needs., • Confidence in decision-making and problem-solving complex issues., • Ability to recognise and respond to distress supporting patients and carers through crisis situations., • Understanding of Advance Care Planning., • Sensitive and effective communication skills particularly in supporting individuals with life-limiting illnesses., • Strong interpersonal skills with the ability to build rapport trust and confidence with colleagues and external professionals., • Experience in managing conflict and difficult emotional situations with empathy and professionalism.
