Job Description Outreach Worker At Father Joe’s Villages, outreach and housing services are a critical first step in helping individuals and families move from homelessness to stability. Our Outreach team builds trust, connects people to essential services, and helps them access safe housing and supportive care. This work directly supports our mission to end homelessness and strengthen the San Diego community. The Role The Outreach Worker engages individuals and families experiencing homelessness in the community and connects them to housing, healthcare, and supportive services. This role combines field outreach, case management, and housing navigation while working closely with community partners and internal teams. You will regularly travel throughout the community to meet clients where they are and help them move toward safe and stable housing. Key Responsibilities • Conduct street and community outreach to engage individuals and families experiencing homelessness and build trusting relationships, • Support clients through the Coordinated Entry System and connect them to shelter, housing, and supportive programs, • Complete intake interviews, needs assessments, and eligibility screenings., • Provide referrals to emergency, interim, and permanent housing resources, • Maintain a caseload ofapproximately 15–20 clients and provide ongoing support., • Develop and update individualized service plans in partnership with clients, • Coordinate referrals to medical care, behavioral health services, substanceusetreatment, and other community resources, • Monitorclient progress and advocate for services that support long-term housing stability, • Maintainaccurateandtimelydocumentation in HMIS and other case management systems, • Complete progress notes and required reporting for client interactions and services provided, • Collaborate with internal teams and community partners to secure housing and supportive resources, • Safely drive agency vehicles to conduct outreach, attend meetings, and transport clients as needed, • Completerequiredtrainingsand compliance activities on time, • Follow agency policies, confidentiality standards, and data integrity requirements, • Associate degree in social work, psychology, sociology, or a related field and two (2) years of relevant experience,or, • High school diploma or GED and at least two (2) years of relevant experience, including lived experience navigating homelessness, housing instability, or related systems., • Basic computer skills, including Microsoft Office or similar programs, • A valid California driver’s license, current registration, insurance, and an acceptable driving record are required due to the essential functions of this role., • Ability tomaintainconfidentiality and manage multiple priorities, • Experience providing outreach, case management, or housing navigation for people experiencing homelessness, • Familiarity with the Coordinated Entry System and local housing resources, • Experience working in street outreach, shelters, supportive housing, behavioral health, or substance use services, • Bilingual or multilingual skills (especially Spanish or other languages commonly spoken in the community), • Experience using HMIS or other case management systems, • Training in Trauma-Informed Care, Motivational Interviewing, Harm Reduction, or Housing First practices, • Experience collaborating with community partners, landlords, or healthcare providers This is a full-time position, typically 40 hours per week, scheduled Monday through Friday from 8:00AM to 4:30PM. Work hours may vary slightly based on program and departmental needs. Compensation and Benefits Pay for this position is based on experience, qualifications, and internal equity. The anticipated starting pay range for this role is $24.00 - $27.61 per hour.
