Job Description Position: Attendant - Scofield Manor Reports To: Scofield Manor Management FLSA Status: Non-Exempt Union: Local 145 Scofield Position Summary: Responsible for arranging work to share the duties of caring for up to 50 residents requiring some help with activities of daily living. This position is responsible for medication management, under the direction of the Nurse. Also responsible for the orderliness of the facility during hours when other administrative staff are off duty. This position reports to a Nurse Manager. This is part-time role. Essential Duties and Responsibilities (include but are not limited to): • Assist residents as necessary with activities of daily living. Taking care to treat each resident with respect and understanding., • Update and review daily logbook, responsible for including any notes that were observed during present shift regarding a resident's condition which may be helpful to personnel on alternating shifts., • Maintain tidiness of bed daily and change bed linens for residents under your care weekly and/or whenever the bed is soiled., • Provide beverages, (I.e. fresh ice water) to residents under your care on both first and second shifts, or pick up pitchers if on the third shift., • Help residents with showers, when necessary or requested. Notify Nurse or Administrator of residents who refuse to shower., • Keep cognizant of all emergency evacuation procedures and fire regulations., • Work with the nurse to ensure that the treatments as prescribed by the attending physicians are being followed for each resident. Take vital signs monthly and as needed., • Ensure that all residents are present during a meal and assist those who need help cutting meat, etc., • Assist new admissions with unpacking and labeling their clothes. Be available to help new residents find their way to the dining room, activity room, etc., • When a resident receives clothes at any time, notify the laundry worker to make sure that all clothes are labeled. (aging population that wish to live in their home, NOT in a nursing home) AND TBI survivors. Salary commensurate with experience. Bachelors AND 3+ years experience required Looking for a Case Management Supervisor to work a part time caseload (see Service Coordination) job duties below. The supervisor is expected to meet any waiver participants prior to the completion of the Revised Service Plan (RSP) developed by a Service Coordinator under their supervision. Any visits that exceed 10 in a month will be assisted by the TLC Directors. The Supervisor will have supervisory meetings with staff on at least a monthly basis, and review and sign-off on all service plans and monthly notes. In addition, they will monitor the quarterly tracking spreadsheet to ensure proper time management by staff. A supervisor MAY maintain an active caseload of waiver participants up to 25. However, this caseload must be reduced from the maximum limit allowed in relation to his/her supervisory responsibilities. The Service Coordinator Supervisor must complete a monthly face to face visit with each TBI waiver participant on their caseload. They must meet with the participant in their home at least quarterly. Job duties The Service Coordinator will also be responsible for: • Formally reviewing, updating and submitting timely Service Plans (ISP, RSP, and addendum) to the RRDS for review (refer to the Service Plan section of this Manual for specific information on Service Plan responsibilities);, • Organizing and facilitating Team Meetings; 30 day meeting and 6 month meeting, • Maintaining records for at least seven years after termination of waiver services;, • Assuring that the UAS is completed annually and meets LOC (Level of Care):, • Maintaining records of waiver transportation as described in the Manual section on Transportation Services;, • Assuring that all waiver service providers and the participant receive a current copy of the most recently approved Service Plan;, • Maintaining knowledge of all approved waiver service providers in their region;, • Conducting in-home visits with the participant no less than once a quarter, virtual visits monthly; and, • Create and Review Housing documents for annual housing audit for participants that use housing subsidy Qualifications needed Persons employed as Service Coordinators must be a/an: 1.
