The dosimetrist is expected to remain current with technical developments, standards of practice, professional issues and changes in regulatory requirements. • Works with technical staff during patient simulation, when required, • Generates treatment plan as per physician’s prescription in consultation with physicist, • Performs independent plan check using the established method of the department, • Transfers approved plan data to the record and verify system and verify data consistency with the approved plan, • Assists in setting up difficult treatment fields in the machine for the first treatment, when required, • Performs weekly chart-check, • Attends weekly chart-round, • Assists preparation and loading of sealed sources for brachytherapy application, • Assists in brachytherapy procedure, • Assists planning and fabrication of blocks/cutouts, when needed, • Performs measurements, when needed, • Assists machine commissioning, acceptance testing, QA, etc., • Assists data acquisition, data modeling and commissioning, etc., • Prepares documentation, forms, etc., • Follows patient’s confidentiality requirements, • Maintains department quality assurance protocols, • Maintains functionality of the planning system and peripherals, • Completes required continued education and training, • Provides emergency dosimetry coverage, when warranted Minimum Education Bachelor of Science in physical or biological sciences is desired. Job DescriptionDescription: Care Management is a service model designed to ensure comprehensive communication and coordination among all caregivers involved in a resident's care. A designated "care manager" takes the lead, overseeing and providing access to the necessary services to address the resident's needs, preventing hospitalizations, maintaining health, and ensuring stability. The Care Manager is responsible for providing and coordinating services for their assigned caseload. This involves guiding program enrollees and their legal guardians through the healthcare system, addressing access issues, building relationships with service providers, and monitoring interventions and outcomes. Requirements: Bilingual in Spanish & English A Must Ø Obtain required Care Management enrollment consent from individuals or legal guardians. Ø Conduct initial and ongoing needs assessments (Child and Adolescent Needs and Strengths; CANS) to determine the most appropriate level of care management. Ø Manage the patient’s Individualized Plan of Care, coordinating services, supporting treatment adherence, and monitoring various needs, including prevention, wellness, medical mental health, care transitions, and social and community services. Ø Meet client contact requirements, including face-to-face visits based on acuity levels. Ø Fulfill Care Management documentation requirements using designated tools like Foothold Care Management HER (FCM) and Health Commerce Systems (HCS) such as UAS-NY and MAPP applications Ø Serve as an advocate for clients within the agency and with external service providers.
