These duties include, but are not limited to, medication/immunization administration, point of care testing (POCT), clinical information abstraction, and patient education that can be in person, via our patient portal, or by phone. The Clinical Coordinator II (Registered Nurse II) will work closely with multiple providers and other clinical and administrative staff to deliver an excellent patient experience that contributes to positive health outcomes for patients and a work culture that represents our values. Responsibilities Administrative/Operations • Assesses and collects data related to patient health status an inputs this medical information into the EMR as instructed by the physician., • Works with the care team to develop the plan and care across the continuum (e.g. inpatient, outpatient, home, skilled nursing facility, schools) which includes assisting with medication management (e.g. medication renewals) inclusive of medical prior authorizations as needed. Provide patient education as required., • Assist with management of urgent and emergent patient situations in conjunction with care team leadership., • Coordinates multidisciplinary health care services and arranges diagnostic tests (i.e. lab, x-ray) as ordered by physicians and performs diagnostic testing, therapeutic, and interventional procedures as needed under physician supervision., • Review and act on test results (lab, x-ray), • Review and manage communications from internal and external "customers" inclusive of in basket messages, updating immunizations, updating health maintenance items., • Ensures infection control compliance within the practice., • Addresses appropriate measures to provide patient and family satisfaction., • May float to other clinics on as needed for operations need., • Assist with the management of patients (adult and pediatric) with multiple chronic health conditions, acute episodes of care, behavioral health challenges while promoting and supporting prevention and wellness. Job Description New Alternatives for Children, Inc. (NAC) is an award-winning not-for-profit agency in Midtown Manhattan, dedicated to serving children and families with medical complexity, chronic health conditions, significant disabilities and behavioral challenges. To assist in achieving this mission, NAC is currently seeking a Bilingual Care Manager to join its Champions for Children's Health Department. The Champions for Children's Health Department is a NYS initiative that serves children on Medicaid (up to age 21) with complex medical and/or behavioral conditions and provides coordinated care management services to improve their care and reduce costs. This is done primarily through the work of a Care Manager who is responsible for ensuring the overall provision and coordination of services to children in an assigned caseload, working in collaboration with an interdisciplinary team.
