You can learn more about us at DEPARTMENT DESCRIPTION OR PROGRAM DESCRIPTION HIRING FOR Our Safe Family Services Program offers supportive services to Transition Age Youth (TAY), ages 18-24 years old, currently in an SDYS program, who have, historically, or are currently experiencing domestic violence. CONTEXT OF THE POSITION The Case Manager will provide comprehensive trauma-informed advocacy services with the CSEC Response Team through the Safe Family Services Program, including, but not limited to, working in collaboration with Child and Family Wellbeing, and other community agencies in addition to providing services to youth who are adolescents and Transitional Aged Youth who have experienced commercial sexual exploitation, human trafficking, and/or Domestic Violence as well as experiencing housing insecurity. The Case Manager is responsible for screening intakes, providing intake assessments, creating a service plan in partnership with the youth, providing crisis intervention, facilitating prevention and intervention groups, conducting outreach activities, providing advocacy, housing navigation and assisting in the day-to-day programmatic functioning. Essential functions include: 1. Provide support services that include accompanying youth to meetings/appointments/court; case management services around stabilization (i.e., housing, employment, education, health, etc.); outreach activities, crisis intervention, and referrals as applicable. Provide transportation when needed., 2. Maintain accurate and timely documentation, data collection and contract compliance of records including file reviews., 3. Conduct emergency and/or immediate outreach in collaboration with Child and Family Wellbeing to prospective service partners in a variety of community settings (Polinsky, Group Homes, Juvenile Hall, In-home, Community Partners, etc.); screen prospective service partners for eligibility to enter program. Engage youth in services for up to 90 days or longer, depending on client need and linkage to long-term resources, of intensive short-term case management., 9. Knowledgeable or willing to be trained in tenant-landlord laws, Fair Housing Laws, and the Transitional Housing Misconduct Act., 11. Seek out new partnerships with low-income housing providers., 12. Job Description Summary The Case Manager 2 for the Rapid Re-Housing Program provides direct and indirect services to a caseload of culturally diverse individuals, Families, and/or TAY (transitional-aged youth) clients, including comprehensive assessment, information and referral, case planning, education, advocacy and other supportive services. Case Manager 2 are responsible for developing professional and empathetic relationships while providing newly housed individuals and families with connections to appropriate programs and resources. The CM2 provides one-on-one case management and develops individualized case plans that promote client progression towards obtaining and maintaining self-sufficiency during program participation, and after the Rapid Rehousing Program is completed. Case Manager 2 are to be comfortable with entering the client’s home on a monthly basis to ensure the unit is being kept up and no breaking of the lease is occurring. Case Manager 2 is responsible for completing psychosocial assessments on clients that are currently in the process of being enrolled. Case Manager 2 also conducts weekly Orientation to clients who are in the process of being enrolled. Essential Functions • Maintains a maximum caseload of 15 households, • Conducts weekly, bi-weekly, and/or monthly meetings with each client, • Conducts at least once per month home visits, grocery runs, and dropping off of other provision of services all throughout San Diego County, • Completes monthly property management engagement to maintain relationship, • Completes monthly budgets with clients and reviews the monthly Rental Assistance Plan, • Creates and maintains a case plan with each individual client. Enters case notes and services into the CSTAR database in a timely manner, • Identifies and supports clients with resources for each client’s individual needs within FJV and in the community, • Conducts psychosocial assessments and directs the RRH weekly Orientation for those clients who are in the process of being enrolled in the Program, • Supports the Supervisor of CM by assisting with onboarding and assisting the other CM1s.
