Job Description Apicha Community Health Center aims to improve community health by providing access to comprehensive primary care, preventive health services, mental health, and supportive services. Program/Department Description The Residential Crisis Support program, or “The Respite,” is a residential mental health crisis, alternative-to-hospitalization program offering short-term Peer-led care in a friendly and supportive home-like environment. Guests are provided with the opportunity to learn and practice recovery and relapse prevention skills to support their current mental health crisis and to provide more effective tools to cope with future crises and avoid hospital-based care. Recovery-oriented services are provided 24-hours a day and include peer support, self-advocacy education and self-help training while supporting guests to maintain their established daily activities such as visiting with friends, attending a doctor or other appointments or going to work or school. Guests are encouraged to participate in the operations of the residence, including working together to prepare communal meals. The maximum length of stay is 28 days. Community Access’s OMH licensed Intensive Crisis Residence (ICR) is inspired by the Soteria model and is designed as an alternative to psychiatric hospitalization for people going through an acute psychiatric crisis and who would benefit from a respite period in a stabilizing, home-like environment with 24-hour peer support and treatment services. The Soteria ICR will work with guests for up to 28 days to help them develop awareness and skills that will allow them to return to the community with an increased ability to manage their mental health concerns. The ICR will follow a Soteria model which emphasizes shared running of a home-like environment, positive expectations of recovery, validation of subjective experiences, the concept of “being with” a person, providing 1:1 support, 24 hours a day, and self-determination. Services are completely voluntary, and guests are encouraged to maintain their daily responsibilities and activities in the community during their stay including work, school, family care, recreation, and wellness appointments. The program works with guests throughout their stay on discharge planning focused on safety and wellness, and provides post-discharge follow up health resource navigation support. Position Overview The Peer Health Navigator’s primary focus is connecting Respite or ICR guests to community resources that can help support them as they move through the crisis and return to their lives in the community. This is a key part of successful discharge planning as it will help guests forge and sustain connections to a variety of professional and community-based supports. These peer navigation services would continue for 90 days after discharge to ensure successful crisis resolution and stability upon their return to the community. We are seeking a qualified healthcare professional to be our Clinical Counselor Outpatient Services who will counsel and aid individuals who have substance use disorders and/or their families/loved ones in a New York State OASAS-certified outpatient treatment program. Utilizing a person-centered and trauma-informed lens, individualized assessment and counseling (telehealth or virtual) services are delivered both in-person and virtually at Project Contact Outpatient at the Center for Recovery and Wellness. Schedule: (3) Weekdays - 10am to 6pm, (2) Weekdays - 12pm to 8pm, and a rotating weekend once a month. Responsibilities • Provide caseloads with weekly individual case management and counseling sessions consistent with the individualized treatment plan for individuals in the Substance Use Treatment and/or Significant Other Services treatment tracks., • Continuously assess the appropriateness of the current level of treatment, corresponding with LOCADTR and utilization review instruments (Substance Use treatment track)., • Coordinate integrated services with medical, mental health, vocational, recovery services, and other social service providers., • Complete detailed progress notes, comprehensive treatment plans, intake/admission assessments, psychosocial assessments, discharge plans, and other required documentation in accordance with OASAS and CARF standards., • Maintain accurate and organized clinical charts, engaging in pre-audits of charts to ensure compliance with OASAS, managed care, and CARF standards., • Send weekly, monthly, and quarterly reports to HRA through the Substance Abuse Tracking and Reporting System., • Specimen collection for clients and monitors for treatment-related issues in order to update the treatment plan (Substance Use treatment track), • Check the toxicology reporting portal daily to ensure any positive toxicology reports are communicated to supervisory staff, documented, and addressed with the client (Substance Use treatment track), • Coordinate integrated services with medical, mental health, housing, vocational, recovery services, and other social service providers., • Facilitation of individual and group-level interventions during other clinician vacations or leave, • Ensure that clients’ annual medical examinations are completed, with associated referrals as needed., • Contact referent and/or family contacts upon admission of new clients in a timely manner, and provide written or oral clinical updates as needed., • Effective advocacy and collateral contacts to empower clients, • Outreach to referral sources to expand the network of clients., • Attend staff meetings and trainings as required, as well as individual and group supervision., • Master's Degree in a mental health discipline (MSW, MHC, MFT, etc), and either licensed in the state of New York, or working under a limited permit and eligible for licensure. Job Description Position Description The Patient Navigator is responsible for providing outreach, engagement, and patient navigation services to individuals in the target population who are contacted through street outreach and community partnerships. They will conduct screenings, provide service orientation, and connect individuals to needed medical and social services, both internally at Harlem United and to external providers as indicated. They will conduct outreach (phone, field) to clients who have been lost to care/follow up and establish ongoing relationships to ensure successful connection to quality medical and social services. This position will start March 1, 2026. Essential Job Functions The following duties are mandatory requirements of the job: • Conduct street outreach, home visits, escorts and other related activities as needed to engage community members in order to link and retain them in care and services., • Provide direct linkage to HIV, HCV, and STI testing, both in the field and on site. Coordinate testing opportunities and events with Harlem United’s testing team., • Ensure access to program by providing targeted outreach and education to members of the community and service providers through presentations and community networking activities., • Conduct individual needs assessment sessions with clients who use drugs and assess for medical services and other health related services.
