Job Description Will cover the Broken Arrow/Catoosa locations Duties & Responsibilities: • Use verbal de-escalation skills, functional assessments, crisis planning/safety planning and/or community resources to assist persons in managing and/or gaining control of a crisis either via telephone/within the clinic within the home or community., • Ensure that all persons assigned to the Broken Arrow and Catoosa clinics discharged from an inpatient psychiatric facility or Community Based Structured Crisis Center (Spring Creek, etc.) receive follow up within 72 hours post discharge via a home visit, phone call or on-site appointment., • The Outreach Specialist is to ensure that persons discharged are scheduled with a CREOKS clinician within one week of discharge and the first available medication management appointment., • Will serve as the responsible care manager (RCM) for persons discharging from the above-mentioned institutions until it is established that the person is engaged in CREOKS clinic-based services with another RCM., • Responsible for completing activities assigned to the HealthLink Outreach Specialist as outlined in the CREOKS Tenure Protocol and assisting their assigned clinic in executing the Tenure Protocol., • The Outreach Specialist is to pursue aggressive outreach to participants who miss any service appointments while subject to the protocol, up to and including phone calls to authorized emergency contacts and visits to the participant's residence., • Provide advocacy, linkage, and referral services on behalf of chronically mentally ill and Most in Need., • Provide intake, screening, referral and case management services., • Provide Care Management, Care Coordination, Comprehensive Transitional Care, Health Promotion, and Referral to Individual and Community Support Services as part of a CREOKS' CCBHC Team-Based care as assigned by the Site Director., • Will assume a whole person approach and will screen for medical care needs, link to medical providers, and provide health promotion education consistent with their level of training and experience., • Assist in maintaining an active liaison with community agencies serving the "high risk" population., • Maintain the clinical records system in accordance with ODMHSAS, OHCA, CARF and CREOKS rules and regulations., • To read and be knowledgeable of all current CREOKS policies and procedures., • Maintain productivity averages of direct service to clientele as set by CREOKS for full time, • Turn in billing and associated documentation as mandated by CREOKS policy., • Responsible for understanding and following all of the administrative, clinical, and programmatic policies and procedures of the agency., • Responsible for attending and presenting in the treatment team staffing conferences and Care Coordination Care Reviews as assigned by Site Director., • Responsible for serving the number of program participants consistent with full time employment as assigned by the Supervisor and maintaining the daily productivity average as set by Administration., • Responsible for billing documentation- to be completed on a daily basis and clinical documentation to be completed preferably, but no later than the beginning of the next work day., • Responsible for necessary interaction with agency program participant families and/or other agencies involved with participants to ensure appropriate therapeutic care., • Responsible for participating in mandatory training such as nonviolent crisis de-escalation, etc., • Responsible to teach Rehabilitative skills that focus on, but are not limited to, skills necessary to perform activities of daily living and successful integration into community life. This service includes educational and supportive services regarding independent living skills, self-care, social skills regarding development, lifestyle changes and recovery practices and principles., • Provide services and supports to all program participants within the community (to include a participant's residence, etc.) consistent with the Care Manager's education, experience, training, and scope of practice., • Minimum of Bachelor’s degree in a mental health related field or a Bachelor’s degree and at least 2 years supervised experienced in a treatment setting., • Maintain current case management certification., • Basic understanding of social and cultural factors influencing human behavior., • Knowledge of various treatment modalities as well as a basic understanding of the impact of physical health on overall wellbeing., • Capability to work as part of a team., • Core competencies in relation to individuals with co-occurring disorders, • Case Management II Certification, • Must complete the “Housing+ Endorsement” training through ODMHSAS, • Become trained and certified as a Wellness Coach according to ODMHSAS standards., • Experience working in integrated care settings or with medical providers is preferredCompany DescriptionCREOKS Health Services is a non-profit organization providing comprehensive health, wellness, and social services.
