Job Description Summary Streetwork Project is an innovative outreach-counseling program for homeless youth in Harlem, which utilizes a harm reduction approach providing comprehensive services to adolescents and young adults impacted by homelessness and involved in street activities (gangs, violence, drug use/sale and survival sex). The Youth Services Navigator will promote access to harm reduction supplies and services at Streetwork’s Drop-In Center, with a focus on LGBTQ youth and youth of color at risk of SUDs Youth Services Navigators (YSN) will facilitate and conduct one-on-one harm reduction goal setting intervention for drug using clients. Youth Services Navigators will provide overdose prevention and response trainings for drug users. The YSN will provide youth seeking harm reduction services with information, counseling and supplies (safer syringe services, sniff kits, safer sex supplies, Naloxone kits and sharps disposal kits), The Youth Services Navigators will also be needed to provide general program services such client monitoring in the milieu, intake and assessment, supportive counseling, referrals, syringe exchange, escorts and the provision of concrete services (meal preparation client, laundry, provision of supplies), and groups. Job Description Summary Streetwork Project is the oldest street outreach program for homeless and street-involved youth in New York City. The program has grown from its beginnings in 1984 as a storefront project to encompass both outreach and comprehensive harm reduction services. The core service is long-term, one-on-one counseling, which is offered in a non-judgmental and holistic manner utilizing a harm reduction approach that focuses on building trust, fostering self-esteem, exploring options and choices, and empowering youth to change their high-risk behaviors. As part of our continuum of services we operate an emergency housing program for homeless youth in NYC, Streetwork Overnight. The Overnight is a 24 bed facility for youth ages 16-20. It is open from 7 pm to 10 am 7 days a week. Casual Youth Advocates work overnight hours to support the youth while they are in our crisis shelter. As part of our continuum of services, we also provide overnight outreach in teams of two seven nights a week (7 pm to 3 am), throughout the five boroughs of New York City. Casual Youth Advocates work overnight hours to meet the most disenfranchised youth while they are in crisis. Casual Youth Advocates work part-time and on-call, filling in for full-time youth advocate absences at the Overnight and Outreach program. These individuals provide comprehensive services to adolescents and young adults impacted by homelessness and involved in street activities (gangs, violence, drug use/sale and survival sex). Services provided by casual youth advocates are the same as full-time youth advocates and include assessment, case planning, case facilitation and management, counseling, referrals, outreach, crisis transportation, daily living services (food, shower, respite), and groups. These Youth Advocates coordinate with Safe Horizon supporting programs and local agencies to provide short- and long-term care necessary to meet the wide range of needs related to homeless adolescents. Job DescriptionSummary: Streetwork Project is an innovative outreach-counseling program for homeless youth in Harlem, which utilizes a comprehensive harm reduction approach that informs all facets of client work. The Housing Advocate will provide advocacy services to adolescents and young adults impacted by homelessness, poverty and mental illness, many of whom are involved in street activities (gangs, violence, drug use/sale and survival sex). The main client-base for this position will be homeless young people with serious and persistent mental illness and/or substance use disorders. The Housing Advocate will work with clients on accessing shelter and permanent housing. The core of the shelter assistance will be supporting clients with referrals to and advocacy within youth shelters (DYCD), domestic violence shelters (DV), and adult homeless shelters (DHS) including both singles and family shelters. The core of the housing assistance will be the completion of supportive housing applications, completion of applications for housing vouchers (including but not limited to Section 8, City FHEPS, HASA), assisting clients who have vouchers in locating and moving into appropriate housing which includes landlord outreach and engagement, and New York City Housing Authority (NYCHA) applications. This will include advocacy with DHS (the NYC Department of Homeless Services) and HRA (the NYC Human Resources Administration) and will necessarily include advocacy for acquiring and navigating cash assistance and disability-income programs. Job Description This position is - 100% onsite Summary: Intake Case Managers are the first point of contact for all clinic clients and provide comprehensive telephonic and walk-in services to individual clients and families impacted by violence (domestic violence, rape, sexual abuse, homicide, etc.). This position comes with excellent training in trauma work as well as a high level of clinic support and supervision. Services provided by Intake Case Managers include safety assessment, risk management, advocacy, phone counseling, information and referrals to resources and providers within and external to Safe Horizon, scheduling and other administrative tasks with prospective clients, as well as engaging clients in a short-term trauma intervention. The Intake Case Manager coordinates with programs within Safe Horizon and local agencies to connect survivors of violence with resources and services best suited to meet their needs. Essential Job Functions: • Learn and integrate a trauma-informed, client-centered approach, using an anti-oppressive lens, into your work with clients., • Greet clients (in person or by phone) in a warm, professional, and trauma-informed way, • Monitor the safety of people and conditions in the waiting area, • Act as a first point of contact for all walk-in and telephonic clients seeking services and re-directing them to appropriate clinic staff when necessary, • Provide intake and assessment for individuals seeking assistance, • Provide screening, assessment, insurance verification, and scheduling services for clients seeking trauma-focused outpatient treatment at a Safe Horizon Counseling Center site, • Provide short-term crisis intervention to victims and survivors of crime., • Provide short-term trauma intervention, • Engage callers in risk assessment and safety planning conversations., • Provide information and referrals for other Safe Horizon services and community-based resources., • Provide assistance with OVS applications., • Provide information regarding accessing health coverage, public assistance, and other entitlements, • Supports the Counseling Center’s transition to , centralized scheduling, open access services, and other quality improvement initiatives, • Request and collect co-payments or self-payments, provide clients receipts, record all payments, and obtain service authorization as necessary, • Provide and document Metrocard distribution, • Cross-train in the activities of receptionist clerk for coverage purposes as necessary, • Performs data entry into a computerized record keeping system., • Maintains cooperative working relationships with other Safe Horizon staff members and other service providers. Basic understanding of computer and systems for managing case records Education: Required: • BA or BS Degree preferred or equivalent relevant work or internship experience within the victim assistance field Experience: Required: • One year of experience working in a social service setting or with survivors of violence preferred Desirable: • Related work or intern experience in a victim assistance setting. Knowledge of effects of victimization and trauma on victims. Knowledge of community resources and options available to victims of crime and abuse. Job Description Summary: Safe Horizon’s Safe Way Forward program provides comprehensive trauma-informed client-centered services to families who are affected by intimate partner violence and are involved in the child welfare system. Our goal is help those who have experienced physical, emotional or other forms of abuse to establish long-term safety and healing from the effects of trauma. Safe Horizon uses a client centered, trauma informed, anti-oppressive approach to supporting clients. Safe Horizon recognizes that families who have experienced intimate partner violence may want to or need to stay in contact with the person causing harm in the family. The Safe Way forward program (SWF) was designed to engage family members with support to assist in healing and explore ways for families to maintain contact where safe to do so. In this model, all participating members of the family are considered clients (survivor, person causing harm, and their children). Hybrid schedule: 2 days on site, 3 days remote. Essential Job Functions: The Trauma Counselor provides both practical trauma intervention and support to domestic violence survivors, their children and the person causing harm (separately) Duties may include some or all of the following: • Administer trauma screenings and trauma assessments to children, survivors of domestic violence and/or persons causing harm in intimate partner relationships, • Develop treatment goals with clients, • Engage clients and provide trauma-focused mental health treatment with fidelity to evidence-based models, • Provide evidence-based trauma-focused treatment through individual, dyadic, and group models, • Work in close partnership with SWF team members and ACS to coordinate service provision for children and families impacted by domestic violence, • Conduct comprehensive assessments for survivors and/or persons causing harm and their families, • Provide crisis intervention and short- and long-term trauma intervention for children, survivors of domestic violence and/or persons causing harm in intimate partner relationships, • Engage in safety assessment and risk management dialogues with clients to follow up on safety plan, • Conduct home visits with clients and their families., • Participate and support clients in family team conferences, • Maintain clear, concise, and timely documentation with all relevant client information including data entry into a computerized record keeping system, • Attend training relevant to position, including learning and consultation on evidence-based treatment models, • Actively participate in individual and group supervision, • Prepare for and participate in regularly scheduled FFP team case review meetings about new and on-going cases, • Maintain open communication with supervisors and team members, • Collaborate with other service providers internally and externally as needed, • Participate in the service team of clinicians and case planners responsible to maintain a mixed caseload of families (including survivors and their children and persons causing harm), • Implement strategies for addressing vicarious trauma Required Skills: • Sensitivity towards and understanding of victimization, especially the experience of survivors of domestic violence and all members of their families including the person causing harm in the family;, • Strong communication and organizational skills; Excellent interpersonal skills, be assertive and well organized; Ability to work under pressure in a calm and reassuring manner;, • Demonstrate the ability to work independently and also work on a team with an appreciation of different points of view, • Ability to work evenings as needed and maintain a flexible work schedule, • Strong oral and written communication skills Qualifications: • Basic understanding of computer and systems for managing case records An advanced degree (LMSW) (license required) or related field., • Prior experience working with children and families impacted by domestic violence, abuse and/or trauma., • Strongly Preferred: Bilingual Spanish/English fluency Experience with family therapy or individual therapy preferred with survivors of domestic violence or persons causing harm in a trauma-informed setting;, • Knowledge of the New York City domestic violence service system, public benefits, child welfare, housing system and other social services.
