Job Description Description LCSW PRIMARY CARE $7500K SIGN ON BONUS Summary: Provides behavioral health consultation to children, adolescents, adults and families with behavioral health needs with the goal of improving psychosocial functioning of Nuvance Health�s patients. Performs assessments and formulates treatment plan and/or transition plans in collaboration with patient, provider, and possible family and multidisciplinary healthcare team. Provides ongoing monitoring and evaluation of plans to meet the individual needs of patients throughout the care continuum. Provides education to patients/families regarding coverage, care options and community resources. Responsibilities: 1.Facilitates the Behavioral Health integration in primary care as the onsite Behavioral Health LCSW performing behavioral health assessments, develop treatment plan and provides brief interventions utilizing various treatment modalities such as CBT, DBT, MI, grief counseling, trauma focused treatment, mindfulness, goal setting, problem solving, solution focused, insight oriented supportive counseling, SBIRT and as needed referrals to HLOC and recommendations for treatment and community services. 2. Behavioral Health Care Coordination and Transition Management - coordinates multiple aspects of care to all appropriate patients to ensure the patient overall health goals improve and their psychosocial wellbeing is sustained., 3. Assesses patient's psychological readiness for transition in collaboration with the healthcare team and informs the team of the available resources based on the patient's needs. Maintains easy flow of communication regarding patient's development and progress to the referring clinician., 4. Maintains current knowledge of hospital and community resources and is a resource for the primary care team., 5. Maintains effective relationships and a positive outlook when interfacing with other departments, multidisciplinary healthcare team as well as patient's/families and community providers., 6. Acts as an intermediary between patients, physicians and community providing advocacy, accurate healthcare information, potential referral services and proper treatment plans., 7. Provides clinical social work interventions which include counseling, bereavement, brief goal focused therapy and utilizes different modalities SBIRT, MI, CBT, DBT, mindfulness, etc. and crisis intervention as well as assessment and intervention in cases involving complex family/patient dynamics. Participates in Simulation Lab trainings and promotes SBIRT and MAT in the PC offices., 8. Maintains confidentiality on relevant issues and information. Job Description Summary: Performs all functions needed to ensure proper storage, preparation and dispensing of medications. Interpret and evaluate physician orders for appropriateness and safety. Reviews, interprets, and verifies medication orders for safety and appropriateness in a timely manner. Conducts patient profile reviews prior to verifying/dispensing. Identifies and resolves basic order entry and prescribing problems. Makes appropriate interventions to ensure patient safety and regulatory compliance. b. Dispenses accurately labeled medications in the appropriate packaging in a timely manner. Understands all drug preparation techniques and can perform or oversee the timely preparation of medications to ensure product and patient safety. c. Ensure all sterile compounded medications and IV solutions are handled and labeled according to applicable regulatory standards and policies. d. Understands and complies with required Pharmacy Department documentation in all areas and can do so in a clear concise manner. e. Understands the purchasing process and assists in inventory control to reduce waste. Can obtain drugs in select emergency situations. f.
